单孔腹腔镜膀胱部分切除术五例并文献复习

摘　　要：目的探讨单孔腹腔镜膀胱部分切除术的临床应用价值及可行性。方法回顾性分析2011年6月至7月间5例单孔腹腔镜膀胱部分切除术患者的临床资料,采用Olympus公司Triport经腹膜外腹腔途径实施单孔腹腔镜下膀胱部分切除术,手术操作包括膀胱部分切除及完全腔内膀胱缝合。结果 5例手术均获成功。手术时间30～50min,平均40min;术中出血10～20ml,平均15ml。清醒后即可饮水。1d后进食;住院时间3～5d。伤口引流管术后第2天拔除。导尿管引流5d后拔除。术后无继发出血、尿漏、切口感染等并发症发生。术后瘢痕不明显,美容效果满意。术后肿瘤病理分期:pT11例,pT22例,pT32例;肿瘤分级:G12例,G22例,G31例。病理报告均为高级别浸润性膀胱尿路上皮癌。结论单孔腹腔镜膀胱切除术手术具有微创、患者恢复快、住院时间短等优点,是一种治疗膀胱肿瘤较为理想的手术方法。Objective To evaluate the clinical value and feasibility of one-trocar laparoscopic partial cysteetomy in bladder cancer. Methods Retrospective analysis 5 cases of bladder cancer underwent ectoperitoneal one-troear laparoseopic partial cystectomy in June-July 2011.We used triport which come from olympus company.The procedure employed consisted in laparoseopic partial cystectomg and exclusive intraeorporeal suture technique. Results The procedure Was completed in all cases.Mean surgical time Was 40 minutes and mean blood loss Was 15mL.There were no significant complications during both intra-and post-operative period.Mean hospitalization period was 3-5 days. drainage catheter removed 5d after operation. There was no hemorrhage, urine leak, wound infection and other complications occurred. Scar is not obvious, cosmetic results were satisfactory. Postoperative pathological stage： pT1 1 case, pT2 2 cases, pT3 2 cases;tumor grade： G1 2 cases, G2 2 cases, G3 1 case. Pathology report： high-level invasive bladder urothelial carcinoma. Conclusions One-Trocar Laparoscopic Partial cystectomy have many advantages including effective, minimally invasive, faster recovery, shorter hospital stay, etc. Its a good treatment for bladder cancer.
