高强度聚焦超声联合曼月乐在慢性盆腔痛治疗中的应用

摘　　要：目的:探讨高强度聚焦超声联合曼月乐治疗慢性盆腔痛的临床效果及安全性。方法:38例患者均为药物治疗无效而行手术和高强度聚焦超声联合曼月乐治疗,每组各19例;对两组患者治疗前和治疗后7天、1月、2月的VAS、血浆P物质和治疗后3、6月满意度进行评价及统计学分析。结果:两组患者于治疗后1、2月VAS及血浆P物质较治疗前显著降低(P<0.05),疼痛明显缓解或消失;手术组治疗后1、2月的VAS及血浆P物质小于高强度聚焦超声联合曼月乐组。治疗后3、6月手术组满意度呈下降趋势,差异有统计学意义(P<0.05);高强度聚焦超声联合曼月乐组的满意度保持水平基本稳定并优于手术组。结论:高强度聚焦超声联合曼月乐治疗慢性盆腔痛的疗效确切,是一种安全、有效方法。Objective：To explore the clinical efficacy and safety of high intensity focused ultrasound combined with Mirena in treatment of chronic pelvic pain. Methods：Thirty-eight patients were treated with surgery and high intensity focused ultrasound combined with Mirena because they were not responsive to pharmacotherapy,19 patients in each group;the scores of visual analogue scale（VAS） and plasma substance P levels before treatment and at 7 days,one month,and two months after treatment,the degrees of satisfaction at three and six months after treatment were evaluated and analyzed retrospectively. Results：The scores of VAS and plasma substance P levels at one month and two months after treatment in the two groups were significantly lower than those before treatment（P0.05）.Pain was relieved obviously or disappeared after treatment.The scores of VAS and plasma substance P levels at one month and two months after treatment in surgery group were lower than those in combined treatment group.At three and six months after treatment,the degrees of satisfaction in surgery group showed decreasing trends,there was significant difference（P0.05）.The degrees of satisfaction in combined treatment group maintained at a stable level,and which were superior to those in surgery group. Conclusion：The clinical efficacy of high intensity focused ultrasound combined with Mirena is good,which is a safe and effective method.
