扩张型心肌病患者斑点追踪量化指标与NYHA心功能分级的相关性研究

摘　　要：目的探讨不同心功能级别扩张型心肌病(DCM)患者的斑点追踪显像(STI)各指标变化趋势,并分析其相关性。方法 45例DCM患者,按照纽约心脏协会(NYHA)心功能分级方法分为L～Ⅱ级、Ⅲ级及Ⅳ级3组。应用STI技术检测左心室收缩期整体峰值纵向应变(GLs)、径向应变(GRs)、圆周应变(GCs)及左室扭转(LVtw),比较不同心功能状态下DCM患者的各参数变化,并与对照组比较。结果较对照组,DCM组GLs、GRs、GCs及LVtw均明显减低(P<0.01);与心功能Ⅰ～Ⅱ级比较,Ⅲ级者LVtw减低(P<0.05),Ⅳ级者GLs、GRs及LVtw均减低(P<0.05、P<0.01);与心功能Ⅲ级比较,Ⅳ级者LVtw明显减低(P<0.01),且LVtw与LVEF具有良好相关性。结论 STI技术可准确检测DCM患者左室扭转及整体收缩功能,重复性好,与NYHA心功能分级相关,可为临床治疗方案的制定和疗效观察提供可靠的定量参数。Objective To investigate speckle-tracking imaging （STI） parameters change trend in Dilated Cardio myopathy（DCM） patients with different cardiac function,and analyzes its relevance. Methods According to the New York Heart association （NYHA） criteria,the cardiac function of forty-five patients with dilated cardiomyopathy,was classified： 11 were class I- II , 18 were class III and 16 were classiv. The systolis parameters including Global longitudi- nal strain（GLs）, circumferential strain （GCs）, radial strain （GRs） and left ventricular twist（LVtw） were calculated by STI. The data between the patient group and the control group were compared. Results GLs,GRs,GCs and LVtw in patients with DCM were smaller than those in the control group（P〈0.01）. GLs.GRs.LVtw in class IV were smaller than those in class I-II ;LVtw in class IV were smaller than those in class i （P〈0.01）. LVtw results were in correlation well with LVEF. Conclusions STI is a fast comprehensive technique for the assessment of Left Ventricular Systolic Function and LVtw in DCM because of its accuracy and its high reproducibility. STI results were in correlation with the classification of NYHA.
