短效与长效促性腺激素释放激素激动剂用于长方案降调节的效果比较

摘　　要：目的:比较同种药物不同剂型促性腺激素释放激素激动剂(GnRH-a)降调节方案促排卵的效果。方法:根据使用不同剂型的GnRH-a降调节,将接受长方案超促排卵治疗的206例患者分成GnRH-a短效组(A组)及GnRH-a长效组(B组),比较两组的降调节效果、促性腺激素(Gn)使用剂量及时间、激素水平、获卵数、优质胚胎数、胚胎冷冻率、妊娠率等。结果:B组Gn用药时间及用量高于A组,分别为(11.62±1.32)d,(10.97±1.20)d]和[(37.87±12.95)支,(29.58±9.93)支,差异有统计学意义(P<0.05);冷冻胚胎率A组高于B组,分别为:45.4%和37.8%,差异有统计学意义(P<0.01)。获卵数、胚胎数、优质胚胎数、受精率、临床妊娠率两组无统计学差异。结论:在控制性超促排卵长方案中应用同种药物不同剂型GnRH激动剂对垂体均有满意的降调,妊娠结局相似,应个体化选择用药方案。Objective： To compare the effects of two different dosage forms of gonadropin releasing hormone agonist（GnRH-a） on the long down regulation protocol of the controlled ovarian hyperstimulation（COH） in vitro fertilization and embryo transfer（IVF-ET）.Methods： Two hundred and six cases received long protocol IVF-ET were divided into two groups： group A,the short-acting group,and group B,the long-acting group.The pituitary down regulation result from the COH,the serum hormone levels,the number of oocytes retrieved and good quality embryos,and the rate of frozen embryos and pregnancy were compared respectively.Results： Group B costs basically much more days（[11.62±1.32] d versus[10.97±1.20] d,P0.05） and gonadotropins（[37.87±12.95] Amps versus[29.58±9.93] Amps,P0.05）.In contrast,the group A surprisingly got higher rate of frozen embryos than group B did,the rates being 45.4% and 37.8% separately（P0.01）.Additionally,the rate of oocytes retrieved,good quality embryos,rates of fertilization and pregnancy had no statistical difference between the two groups.Conclusion： The effect of two different dosage forms of GnRH-a on pituitary down regulation and pregnancy outcome was similar.Individualized drug administration should be paid more attention in clinic.
