脉冲组织多普勒对甲状腺亢进性心脏病右心室舒张功能的评价

摘　　要：目的应用脉冲组织多普勒技术测定三尖瓣环运动频谱峰值速度评价甲亢患者右心室舒张功能的价值。方法选取甲亢性心脏病患者和正常人各20例,先进行传统超声心动图检查,测三尖瓣口A峰、E峰、A/E值,再在脉冲组织多普勒模式下测右室游离壁和室间隔处三尖瓣环Em、Am、Em/Am。结果对照组、甲亢组三尖瓣口血流均为E>A,E/A>1,两组比较差异无统计学意义(P>0.05)。对照组,室间隔及右室游离壁瓣环处的Em>Am,Em/Am>1;甲亢组,室间隔及右室游离壁瓣环处的Em<Am,Em/Am<1,两组比较差异有统计学意义(P<0.05)。结论传统超声多普勒无法正确评价甲亢患者右室舒张功能,而脉冲组织多普勒可以早期发现甲亢患者右心室舒张功能减低。Objective To evaluate value of diastolic function of right ventricle by using pulse wave Doppler tissue（PWDT） to check velocity of peak value of tricuspid valve annulus moving.Methods 20 patients with hyperthyroid heart disease as research group and 20 normal subjects as control group,all subjects were checked value of A peak,E peak,A/E of RVFW and ventricular septum by using PWDT and traditional Doppler.Results The blood flow value on tricuspid orifice was E〉A,E/A〉1 in each group,and there was no difference between them（P〉0.05）.In controls the blood flow value on tricuspid orifice was Em〉Am and Em/Am〉1,but in research group,the blood flow value on tricuspid orifice was Em〉Am and Em/Am〉1.There was significant difference between them（P〈0.05）.Conclusion Traditional Doppler can not correctly evaluate diastolic function of right ventricle for patients with hyperthyroid heart disease,but PWDT can decrease diastolic function of right ventricle at early stage.
