输尿管镜下钬激光治疗输尿管狭窄73例分析

摘　　要：目的总结和分析经输尿管镜钬激光治疗输尿管狭窄的疗效。方法经输尿管镜逆行至狭窄处,钬激光内切开输尿管狭窄处,留置F6双J管8～12周。拔除双J管后,比较治疗前、后症状肾小球滤过率(GFR)了解术侧肾功能改善情况,B超、静脉尿路造影(IVP+KUB)及CT检查了解治疗前、后影像学改变。术后随访2年。结果术后患者症状明显改善49例(67.1%),GFR显示术侧肾功能明显改善,B超检查示术侧肾积水明显改善,IVP+KUB检查示术侧造影剂排泄通畅,CT检查示术侧肾积水改善、狭窄处恢复通畅。12例失访,12例无效。结论输尿管镜下钬激光内切开治疗输尿管狭窄效果良好,值得临床推广应用。Objective To summarise the therapy effect of holmium laser to treat ureterostenosis through ureteroscope.Methods We used holmium laser to treat ureterostenosis through ureteroscope.Laser cut urethral in narrow ureter,a french 6 double J ureteral stent was left for 8-12 weeks.After pull out double J ureteral stent,we compared the symptom and the the renal function by glomerular filtration rate（GFR） before and after treatment.We compared the change of B ultrasound,intravenous pyelogram ＋ kidney ureter bladder film（IVP＋KUB）,computed tomography（CT） before and after treatment.Cases were followed up for 2 years.Results The symptom was mitigated obviously in 49（67.1%） cases,GFR showed the the operation side of kidney function was significant improved cases,B ultrasound showed the operation side of hydronephrosis obviously relief,IVP＋KUB showed the operation side contrast agents discharge flow,CT showed the operation side of hydronephrosis relief.12 cases were lost to follow-up,12 cases were invalid.Conclusion Use holmium laser to treat ureterostenosis through ureteroscope get the hight therapy effect,it is worth for clinical application.
