切吸联合臭氧治疗腰椎间盘突出症的早期疗效

摘　　要：目的观察经皮腰椎间盘切吸术(PLD)联合臭氧治疗腰椎间盘突出症的早期疗效。方法 L3—4和L4—5椎间盘突出者,在C臂机透视下,俯卧位,经侧后方入路引入直径5.6 mm工作套管;L5—S1椎间盘突出者,采用侧卧位,腰部垫高,患侧下肢过伸体位进行穿刺入路。均摘除髓核,直至无明显髓核钳夹出后,于盘内及椎间孔附近各注入臭氧10 mL。结果 60例患者穿刺及技术操作成功率100%,随访3个月以上,总有效率92%。结论早期PLD与臭氧注射术联合应用治疗腰椎间盘突出症是一种安全、有效、损伤小的治疗方法。Objective It is to observe the clinical effect of percutaneous lumbar discectomy（PLD） combined with O3 injection on lubar intervertebral disc protrusion in early stages.Methods Prone position on unhealthy side of L3—4,L4—5,under the direction of C-arm,a working pipe was inserted from the back lateral way;While L5—S1 protrusions the main lumbar discs were treated with lateral decubitus position on the affected side of lower extremity hyperextension postures.Extirpation of nucleus pulposus was done and then O3 was 10 mL injected into the lumbar disc space and near by area of the intervertebral foramina.Results Puncture and technical operation were performed successfully in all the patients.At three months or more follow-up,the patients with excellent results in 30,effective in 25 and invalid in 5.The total effective rate was 92%.Conclusion PLD combined with O3 injection is the advantageous supplement mutually possessing safety and effective.
