摘 要:目的了解医院不动杆菌属细菌的分布及药敏率,指导临床合理使用抗菌药物。方法对2008年2月-2010年4月送检的2688份痰标本分离出的127株不动杆菌属细菌,比较检出鲍氏不动杆菌(ABA)科室的分布及药敏结果。结果 2688例痰标本共检出ABA 119株,鲁氏不动杆菌8株,检出率分别为4.4%和0.3%;ABA对14种常用抗菌药物普遍耐药,平均耐药率高达80.9%,仅对头孢哌酮/舒巴坦敏感率为65.5%;而鲁氏不动杆菌耐药率较低,平均耐药率为22.1%;ABA主要检出科室为:ICU、神经外科、神经内科、呼吸内科,这些科室的多药耐药株、泛耐药株例数显著高于其他科室(P<0.05)。结论痰标本中分离的不动杆菌属主要以ABA为主,随着抗菌药物的广泛应用,其敏感性发生了巨大变化,其中多药耐药株、泛耐药株占有很高的比例,临床已经面临无药可治的局面,加强ABA耐药机制的深入研究,控制医院感染的蔓延刻不容缓。OBJECTIVE To investigate the drug susceptibility and distribution of Acinetobacter,so as to provide evidence for the rational application of antimicrobial agents.METHODS From Feb.2008 to Apr.2010,totally 127 strains of Acinetobacter were isolated from 2688 sputum specimens,the distribution and drug susceptibility testing results of A.baumannii(ABA) were compared.RESULTS A total of 119 strains of A.baumannii and 8 strains of A.lwoffi were isolated from sputum samples,with the detection rates of 4.4% and 0.3%,respectively.A.baumannii was resistant to 14 common antibiotics,with the average rate of drug resistance reaching 80.9%.ABA was only susceptible to cefoperazone/sulbactam with the susceptibility rate of 65.5%;the resistance rate of A.lwoffi was relatively low with the average resistance rate of 22.1%.ABA isolates were mainly detected from ICU,neurosurgery department,neurology department,and department of respiratory medicine.The multidrug-resistant isolates and pan-drug resistant isolates were significantly more prevalent in these departments than in other departments(P〈0.05)CONCLUSION A.baumannii is the predominant Acinetobacter isolated from sputum.As the antibiotics are widely used,its drug susceptibility has been dramatically changed.There is a high percentage of multidrug-resistant bacteria and pan-drug resistant bacteria.It is urgent to control the spread of nosocomial infections through further study on resistance mechanism of ABA.
