玻璃酸钠关节腔注射配合小针刀松解治疗膝骨关节炎疗效观察

摘　　要：目的:观察玻璃酸钠关节腔注射配合小针刀松解治疗膝骨关节炎的临床疗效。方法:将符合要求的170例患者随机分为2组,治疗组86例,对照组84例。治疗组采用玻璃酸钠关节腔注射结合小针刀松解治疗;对照组口服芬必得胶囊,局部外涂扶他林乳胶剂。分别于治疗前和治疗3周后采用Lequesne膝骨关节炎严重性和活动性指数评定患者的膝关节功能,同时于治疗结束后参照《中医病证诊断疗效标准》中骨痹的疗效标准判定2组患者的临床疗效。结果:①治疗前2组患者膝关节功能比较,差异无统计学意义(t=0.958,P=0.836);治疗组膝关节功能评分治疗前后差值大于对照组(t=22.921,P=0.005)。②治疗组临床疗效优于对照组[治疗组95%CI为(0.582 0,0.690 2),R治疗组=0.636 1;对照组95%CI为(0.399 2,0.525 2),R对照组=0.462 2]。结论:玻璃酸钠关节腔注射配合小针刀松解能明显改善膝骨关节炎患者的膝关节功能,疗效优于口服芬必得胶囊配合外涂扶他林乳胶剂疗法,是治疗膝骨关节炎的有效方法。Objective：To observe the clinical curative effect of intra-articular injection of sodium hyaluronate combined with soft tissue release with small knife needle on knee osteoarthritis（KOA）.Methods：One hundred and seventy patients met the criteria were randomly divided into 2 groups,86 cases in the treatment group,while the others in the control group.Patients in the treatment group were administrated with intra-articular injection of sodium hyaluronate combined with soft tissue release with small knife needle,while the others in the control group were administrated with oral application of Fenbid capsule combined with local external application of Votalin emulsion.Knee joint functions of patients were evaluated according to Lequesne index of KOA severity and activity before the treatment and after 3 weeks＇ treatment respectively,and the clinical curative effects were determined according to the criterion for therapeutical effect evaluation of BONE ARTHRALGIA specified in Standard for diagnosis and therapeutic effectiveness evaluation of traditional Chinese medicine syndromes after the end of treatment.Results：①There was no statistical difference in knee joint functions between the 2 groups before the treatment（t=0.958,P=0.836）.The difference of knee joint functional scores before and after the treatment of treatment group were greater than that of control group（t=22.921,P=0.005）.②The clinical curative effect of treatment group was better than that of control group[treatment group：the 95% confidence interval was（0.582 0,0.690 2）,R=0.636 1;control group：the 95% confidence interval was（0.399 2,0.525 2）,R=0.462 2].Conclusion：The method of intra-articular injection of sodium hyaluronate combined with soft tissue release with small knife needle performs well on KOA for the obvious improvement of knee joint functions,and it has better curative effects compared with the method of oral application of Fenbid capsule combined with local external application of Votalin emulsion.
