胎儿永久性右脐静脉的产前超声诊断及预后评价

摘　　要：目的探讨胎儿永久性右脐静脉(PRUV)的产前超声诊断及预后。方法收集我院超声检查发现的PRUV胎儿92胎,分析PRUV胎儿合并畸形情况及预后。结果 PRUV胎儿检出率为0.16%(92/58 946)。孤立性PRUV共74胎(74/92,80.43%),49胎获得随访且均预后良好;合并畸形的PRUV共18胎(18/92,19.57%),6胎预后良好,11胎引产,1胎胎死宫内。结论产前超声可诊断胎儿PRUV。孤立性PRUV胎儿预后良好。对于合并多系统畸形的PRUV胎儿,应结合畸形严重程度及染色体检查结果来评价预后情况。Objective To observe the value of prenatal ultrasonic diagnosis and prognosis for fetal persistent right umbilical vein（PRUV）.Methods Ninety-two PRUV fetuses detected with ultrasound were selected,and the combined anomalies and prognosis were analyzed.Results The incidence of PRUV was 0.16%（92/58 946）.Seventy-four fetuses（74/92,80.43%） were isolated PRUV,among them 49 fetuses had favorable prognosis.Eighteen fetuses（18/92,19.57%） combined with malformation,among them 6 fetuses had favorable prognosis,11 underwent induced labour,while 1 dead in uterus.Conclusion Fetal PRUV can be diagnosed with prenatal ultrasound.Isolated PRUV has favorable prognosis.Prognosis of PRUV fetuses combined with other malformationa should be evaluated according to the severity of malformations and the karyotyping of chromosome.
