摘　　要：目的探讨以皮肤软组织扩张术为主的联合方案修复头面部大面积瘢痕畸形的方法和疗效。方法自2003年1月至2011年6月，共治疗26例头面部瘢痕患者，瘢痕面积3cm×6cm-10cm×15em。根据瘢痕特点，采用不同的皮肤扩张术方法，扩张正常头皮组织和部分额面部皮肤，然后切除瘢痕，遗留缺损由扩张皮瓣采用推进或旋转方式修复。部分严重患者，还须结合邻近皮瓣转移或游离皮瓣修复。结果绝大部分患者瘢痕畸形得到完全修复，术后随访1-2年，获得满意疗效。结论根据瘢痕畸形的不同特点，采用不同的皮肤扩张术对头面部瘢痕进行修复，可获得良好的临床效果。Objective To explore the clinical method and effect of multiple therapies combined with skin soft tissue expansion on the repair of cranial and maxillofacial scar deformities. Methods From January 2003 to June 2011, totally 26 patients with scar deformities ranged from 3 cm x6 cm to 10 cm x 15 cm on the head and the face were treated in this study. According to different clinical feature, different skin soft tissue expansion methods were performed to expand the skin and soft tissue at cranial and facial regions. Advanced or reversed skin flaps were used to cover the defects after scars resection. Transferred or free grafted skin flaps were performed for the cases with severe problems. Results After 1 to 2 years follow-up, majority got satisfactory outcomes with fully repair of the scar deformities. Conclusion It could obtain optimal clinical effect by using multiple therapies com- bined with soft tissue expansion according to different clinical feature of cranial and maxillofacial scar deformities.
