腰麻联合硬膜外麻醉与单纯硬膜外麻醉用于剖宫产术的麻醉效果比较

摘　　要：目的探讨腰麻联合硬膜外麻醉应用于剖宫产术的临床效果及意义。方法将260例在剖宫产产妇随机分为观察组和对照组各130例,观察组给予腰麻联合硬膜外麻醉,对照组给予单纯硬膜外麻醉,观察两组麻醉效果、新生儿Apgar评分及不良反应情况。结果观察组麻醉起效时间、麻醉恢复时间均低于对照组,组间比较有显著差异(P<0.05);观察组麻醉优良率为100%,明显高于对照组的84.61%,组间比较有显著差异(P<0.05);两组新生儿Apgar评分及不良反应发生率比较无显著差异(P>0.05)。结论腰麻联合硬膜外麻醉具有镇痛完全、起效快、肌松好等诸多优点,是目前剖宫产术麻醉方案的最佳选择,值得临床推广应用。Objective To investigate the clinical effects and significance of spinal combined with epidural anesthesia anesthetic method for cesarean section.Methods 260 cases with cesarean section were randomly divided into the observation group and the control group,with 130 patients in each group,observation group received combined spinal and epidural anesthesia,the control group received epidural anesthesia alone,anesthesia,neonatal Apgar scores and adverse reactions were observed.Results The onset time of anesthesia and the anesthesia recovery time were lower than those of the control group,which were significantly different（P〈0.05）;anesthesia excellent rate of the observation group was 100%,significantly higher than that of the control group,84.61%,which had significant differences（P〈0.05）;Apgar score and incidence of adverse reactions of the two groups showed no significant difference（P〉0.05）.Conclusion The combined spinal and epidural anesthesia has full analgesia effect,rapid onset,good muscle relaxation and many other advantages,is the the best choice of anesthesia program for cesarean section,and worthy of clinical application.
