老年高血压病患者血压变异性对血小板体积、高敏C反应蛋白的影响

摘　　要：目的探讨老年高血压病患者血压变异性(BPV)对平均血小板体积(MPV)及高敏C反应蛋白(hsCRP)的影响。方法入选初诊的老年高血压病患者120例,行24h动态血压监测(ABPM),根据24h收缩压标准差(SSD)第50百分位数(P50),分为高BPV组和低BPV组,每组60例。另入选血压正常的40例老年健康者作为对照组。检测各组患者BPV、MPV及hsCRP水平,并进行比较分析。结果与对照组相比,高血压病患者BPV值显著增加(P<0.05)。高血压病患者MPV、hsCRP水平与对照组(P<0.05),高BPV组MPV、hsCRP水平与低BPV组(P<0.05)间的差异均有显著统计学意义。MPV值、hsCRP值与SSD的呈正相关(r分别为0.592、0.795,P<0.05)。结论老年高血压病患者的血压变异对MPV、hsCRP显著影响,可促进血小板活化及炎症反应。Objective To discuss the influences of blood pressure variability（BPV） on mean platelet volume（MPV） and high-sensitivity C-reactive protein（hsCRP） in senile hypertensive patients.Methods The senile hypertensive patients with tentative diagnosis were selected（n=120） and given 24-hour ambulatory blood pressure monitoring（ABPM）.All patients were divided into high BPV group and low BPV group（each n=60） according to the 50th percentile（P50） of 24-hour systolic pressure standard deviation（SSD）.Other senile healthy persons with normal blood pressure were chosen as the control group.The levels of BPV,MPV and hsCRP were detected and compared among all groups.Results Compared with the control group,BPV increased significantly in hypertensive patients（P0.05）.There were significant and statistical differences in MPV and hsCRP between hypertensive patients and control group（P0.05）,and between high BPV group and low BPV group（P0.05）.The values of MPV and hsCRP were correlated to SSD（r=0.592,r=0.795,P0.05）.Conclusion The variation of blood pressure has a significant influence on the levels of MPV and hsCRP,and can promote platelet activation and inflammatory reaction.
