配偶间幽门螺杆菌感染的调查研究

摘　　要：目的调查配偶间幽门螺杆菌感染状况以及确定配偶间是否存在幽门螺杆菌的传播,并分析影响配偶间H.pylori传播的因素。方法使用13C-尿素呼气试验对武汉市206例胃炎患者及其配偶进行H.pylori感染检测,同时使用ELISA法检测同一地区102例18～22岁未婚志愿者的H.pylori感染状况。结果 206例胃炎患者的H.pylori感染率为60.19%(124/206),124例H.pylori感染的胃炎患者的配偶的感染率为56.45%(70/124),82例未感染患者的配偶H.pylori阳性率为37.80%(31/82)。拥有H.pylori阳性的配偶是H.pylori感染的危险因素(OR=2.13,95%可信区间:1.21～3.75)。未婚志愿者的H.pylori阳性率(34.31%)显著低于H.pylori(+)胃炎患者配偶的感染率(χ2=11.026,P=0.001<0.05)。胃炎患者的呕吐症状和抽水马桶的使用与H.pylori(+)患者的配偶感染相关(P<0.05)。结论 H.pylori可以在夫妻之间传播,胃炎患者的呕吐和反流症状以及家庭不良卫生状况是H.pylori(+)者的配偶感染的危险因素。Objective To investigate the status of Helicobacter pylori infection in couples and clarify the route of H. pylori transmission in couples. Methods A cross-sectional study was performed in two groups of subjects. Carbon-13- urea breath test was performed in 206 patients with gastritis and their partners to evaluate the present infection status. Another group of 102 unmarried adults whose age were 18 to 22 were tested by ELISA. Results The prevalence of H. pylori infection in 206 patients with gastritis was 60.19% （124/206）, and 56.45% （70/124） of the partners of 124H. priori infected patients were found to be infected. The H. pylori positivity rate of the partners of 82 uninfected patients was 37.80% （31/82）. The infected partner was the risk factor for H. priori infection （OR = 2.13,95% confidence in- terval = 1.21 - 3.75）. The H. priori prevalence of the unmarried group was notable lower than the partners of 124 H. priori infected patients （X^2 = 11. 026, P = 0. 001 ）. The symptom of vomiting and regurgitation of the gastritis patients and the using of flush toilet in the house were related to the partners infection status. Conclusion H. priori transmission occur in couples in city and the infected partner is the risk factor for H. pylori infection.
