摘　　要：目的观察5-氨基酮戊酸光动力疗法(ALA-PDT)治疗宫颈高危型HPV感染的临床疗效。方法将75例病程>1年的宫颈高危型HPV(HR-HPV)感染患者随机分为2组,治疗组40例,予10%ALA温敏凝胶外敷3h后行635nm的红光照射,每两周治疗1次,共3次,治疗结束后进行随访;对照组35例,不进行任何治疗,直接进行随访。对两组患者均在第3,6,9个月时进行随访,且第3和9个月时用HPVDNA杂交捕获试验和基因芯片法检测HR-HPV。结果治疗组在治疗结束后第3个月时有27例宫颈HR-HPV DNA出现转阴,占67.50%,第9个月时又增加2例转阴,对照组分别为8例(自然转阴率为22.86%)和5例,治疗组阴转率(72.50%)明显高于对照组(37.14%),差异有统计学意义(P<0.05)。两组均未见严重不良反应。结论 ALA-PDT可有效清除宫颈上皮细胞中的HR-HPV DNA,安全有效,它可作为早期干预和预防宫颈癌发生的一种方法。Objective To investigate the efficacy of 5-Aminolaevulinic acid-photodynamic therapy （ALA-PDT） for cer- vical high-risk HPV infection. Methods The hybrid capture 2 HPV DNA assay and gene chip techniques were performed for the high-risk HPV（HR-HPV） DNA typing in 75 patients with HR-HPV infection of cer- vix. Forty patients with cervical high-risk HPV infection were applied topically 10% ALA thermal gel fol- lowed by PDT in the treatment group. The treatment was repeated 14 days later. Totally 3 cycles were given. Thirty-five patients with cervical high-risk HPV infection received no treatment in the control group. All pa- tients were followed up at 3,6,9 months. HR-HPV DNA test by hybridization at 3,9month. Results Twen- ty-seven of 40 （67.50%） cases achieved complete remission with three cycles of ALA-PDT and 8 of 35 （ 22.86% ） cases with no treatment at 3 month, respectively. Complete remission rate were 72.50% and 37. 14% with ALA-PDT and no treatment, respectively at 9 month while there was significant difference （P 〈 0. 05 ）. Conclusion Topical ALA-PDT is effective,less recurrent, safety, and well tolerated treatment for cer- vical HR-HPV infection.
