摘  要:目的探讨成人社区获得性肺炎(CAP)患者血浆可溶性P-选择素(sPs)浓度的演变趋势,结合炎症指标C反应蛋白(CRP)、血小板(PLT)的变化,了解sPs的临床作用。方法将CAP患者分为2组:A组为36例CAP并发继发性血小板增多症(ST)患者,B组为36例无ST的CAP患者,分别检测观察对象5次的sPs浓度、CRP、PLT的数值,即入院0h、24h、48h、72h和痊愈出院时;对照组为180名健康成人,分别检测sPs浓度、CRP和PLT。结果A组sPs、CRP、PLT均较B组高(P〈0.01),三项指标间有正相关关系,sPs与CRP相关性最好(r=0.749,P〈0.01)。结论结合CRP、PLT等炎症指标,连续检测CAP患者sPs浓度对CAP病情的判断、治疗和预后可能有一定的临床价值。Objective To understand the soluble P-selectin clinical effects, and analyse the evolution tendency of the plasma soluble P-selectin (sPs) concentration of adult community-acquired pneumonia (CAP) patients. The inflammatory biomarkers changes of C-reactive protein (CRP) and platelet (PLT) were analysed at the same time. Methods The patients were divided into two groups: group A:CAP with secondary thrombocytosis (ST) ( n =36);group B:CAP simply ( n =36). The sPs concentration,the CRP and the PLT were detected when the patients were admitted, and 24 h,48 h,72 h later, and the patients recovered to discharge. In control group ( n =180), the sPs concentration, the CRP and the PLT were detected once. Results The PLT,sPs and CRP of the patients in group A were higher than those in group B (P 〈 0.01). There were positive correlation in the three indexes, the best correlation was between sPs and CRP (r = 0. 749, P 〈 0.01 ). Conclusions Combined with the inflammation biomarkers including CRP and PLT,continuous detecting the concentration of sPs may have some useful clinical value in the judgement,treatment and prognosis of the CAP patients.
