摘　　要：报告1例以眼损害为首发表现且无皮疹的系统性红斑狼疮。患者女,27岁。因双眼突发视物模糊1周就诊。眼科检查:双眼视力为指数15 cm,眼底检查及眼底荧光造影见大量棉绒斑及严重的视网膜动脉阻塞。皮肤科检查:面部未见蝶形红斑。结合该患者具有口腔溃疡、关节痛、浆膜炎、全血细胞减少、蛋白尿、抗核抗体阳性及免疫学异常[抗双链(ds)DNA抗体、抗Sm抗体、抗心磷脂抗体阳性],诊断为系统性红斑狼疮。A 27-year-old woman presented with sudden blurred vision for one week. The ophthalmological examination revealed that her visual acuity of both eyes was counting fingers at a distance of 15cm. The fundus examinations showed bilateral multiple cotton wool spots and the fundus fluorescein angiography （FFA） revealed severe occlusion of the retinal artery. Besides the above ocular manifestations, the patients was found to have oral ulcer, arthralgia and serositis, however, there was no malar rash on the face and any other skin lesions on the body. Laboratory studies showed pancytopenia, proteinuria, and positive antinuclear antibodies including anti-dsDNA antibody, anti-Sm antibody and antiphospholipid antibody.
