摘　　要：目的探讨肺移植术后并发症的多层螺旋CT（MSCT）表现。方法18例终末剐肺病您行接受单肺或双肺移批．肺移植术后常夫胸部MSCT平扫后．再对扫描图像进行薄以重建及多平晰亘建（MPR）．观察移怕肺实质变化的细做结构和支气管并发痛等。结果18例患背中，移植肺一灌注水肿7例，CT表脱为毛玻璃片样影及胸腔积液、硒限性忡宴变。急性排斥反应5例,CT表小为毛玻璃片样影、小叶问隔增厚、支气管扩张伴管嚷增厚、胸腔干积液。慢性排斥反心1例．CT表脱为毛玻璃片样影．支气管或川芷气筲扩张伴符雌增厚、小叶阃隔增厚、胸腔积液。吻介n狭窄5冽。所仃患者移植后肺部均出现不同程度的感染。结论MSCT肺簿层重建图像可清楚显示终末期肺移植术后并发症的肺内改变并准确对其评估：MSCT可作为肺移植术后定期随访复查的重要检查工具。Objective To evaluate the muirlslice spiral computed tomography （MSCT） findings of the postoperative complications following lung transplantation. Methods 18 patients with end stage pulmonary diseases received single and bilateral hmgs transplan tation underwent CT scans,and the original images were reconstructed with thin section and multiplanar reconstruction （MPR）. The appearances of pulmonary parenchyma and bronchial anastomosis complications were observed. Results Among the 18 lungtrans plant recipients, 7 cases presented reperfusion lung edema, which appeared as groundglass opacities accompanied by interlohular sep tal thickening, ateleetasis and pleural effusion. 5 cases presented acute rejection,CT scans showed ground glass opacities, interlobu lar septal thickening, minimal bronchial dilatation and pleural effusion. One case presented chronic rejection, CT scans showed groundglass opacities, seplal thickening （interlobular and intralobular） ,bronchiectasis accompanied with wall thickening,and pleural effusion. 5 recipients presented bronchial anastomotic slenosis. All recipients developed various pulmonary infection. Conclusion MSCT and thinsection CT scans can clearly demonstrated and evaluate the lung changes of the postoperative complications after lung transplantation. MSCT play an important role in follow up of the postoperative complications after hmg transplantation.
