肢体挤压伤严重程度评分系统比较及对预后的评估意义

摘　　要：目的探讨肢体挤压伤严重程度MESS评分、Open Fracture分类及Gustilo分级系统对挤压伤患者预后的评估意义。方法分别采用MESS评分和Gustilo分级对我院急诊科收治的54例挤压伤患者进行肢体挤压伤严重程度分级,应用Open Fracture分类对其中19处伴有开放性骨折的挤压伤患者进行分类,并追踪患者的预后,统计分析3种评分与患者预后的相关性。结果 MESS评分和Gustilo分级可以指导临床医生对挤压伤患者的预后进行判断,MESS评分优于Gustilo分级,而Open Fracture分类不适用于临床伴开放性骨折的挤压伤患者预后的评估。结论对于四肢挤压伤患者预后的评估,MESS评分和Gustlilo分级可为临床医生提供参考,特别在保肢与截肢早期评估方面选用MESS评分可能较好。Objective To evaluate the value of mangled extremity severity score（MESS）, Classification of open fractures and gustilo classification in prognosis of crush limb injury. Method A retrospective study was carried out on 54 cases of crush limb injuries classificated by MESS and gustilo classification in emergency department of our hospital. 19 extremities of crush limb injuries and subsequent open fractures classificated by classification of open fractures. Prognoses were followed up, and the correlation with three classifications and patients＂ prognoses were statistically analyzed. Result MESS and Gustilo classification are important references for evaluation of the outcome of crush limb injuries,MESS is more accurate than gustilo classification,however open fracture classification cannot be applied to evaluate the the outcome of crush limb injury and subsequent open fractures patients. Conclusion MESS and gustilo classification are important references for evaluation of the outcome of crush limb injuries. The value of MESS is better in early predicting the reservation and amputation of crush limb injuries.
