微创经皮钢板接骨术结合肱骨近端锁定钢板治疗老年肱骨近端骨折疗效分析

摘　　要：目的 探讨采用微创经皮钢板内固定技术（MIPPO）结合肱骨近端锁定钢板（LPHP）治疗老年肱骨近端骨折的疗效.方法 23例肱骨近端骨折患者,男9例,女14例,年龄65～87岁,平均年龄71.3岁,接受MIPPO结合LPHP内固定治疗.术后随访10个月（6～12个月）,平均10个月.结果 Neer氏评分示肩关节功能优11例,良10例,可2例,总优良率为91.3%,三角肌肌力5级.无感染及肱骨头缺血性坏死发生.结论 MIPPO结合LPHP治疗老年性肱骨近端骨折,具有创伤小、固定牢固、能够早期功能锻炼的优点,治疗效果良好.Objective To investigate the effects of minimally invasive percutaneous plate osteosynthesis （MIPPO） combined with locking proximal humerus plate （LPHP）in treatment of proximal humerus fracture of aged patients. Methods Twenty-three patients of proximal humerus fracture, 9 males and 14 females, aged 71.3 （65- 87）, were treated by MIPPO combined with LPHP and then followed up for 10 （6 - 20） months. Results The functional results of the shoulder, according to Neer＇ s scoring system, were classified as excellent in 11 cases, good in 10 cases, and fair in 2 cases, with an average score of 91.3%. The muscle strength of the deltoid muscles was grade 5. No cases of infection and ischemic necrosis of humeral head were found. Conclusion With minimal invasion and stable fixation, and conducive to early functional exercise, MIPPO combined with LPHP is effective in the treatment of proximal humerus fracture in aged patients.
