摘 要:目的:了解本地区耐甲氧西林表皮葡萄球菌(MRSE)的耐药基因存在状况,并对MRSE菌株进行基因同源性分型,为临床治疗及感染控制提供依据。方法:应用聚合酶链反应(PCR)对39株MRSE的6类耐药基因进行检测,并采用随机扩增多态性DNA(RAPD)技术对菌株进行基因同源性分型。结果:39株MRSE的mecA、ermB、ermC、msrA、tetM、tetK、Aac(6')/aph(2″)、aph(3')-III和mupA耐药基因的阳性率分别为100.0%(39/39)、12.8%(5/39)、94.9%(37/39)、43.6%(17/39)、5.1%(2/39)、2.6%(1/39)、82.1%(32/39)、25.6%(10/39)和12.8%(5/39),未检测出er-mA、tetO、tetL、ant(6')-I、vanA和vanB耐药基因;RAPD将39株MRSE分为11型,其中A型占48.7%(19/39)。结论:本地区的MRSE存在高比例的mecA、ermC、Aac(6')/aph(2″)耐药基因,RAPD分析提示MRSE的流行株以A型为主。Objective:To study the conditions of drug resistant genes and the homologous strains in methicillin-resistant Staphylococcus epidermidis(MRSE) in our region,in order to guide clinical treatment and infection control.Methods: Six type of resistant genes were detected by polymerase chain reaction(PCR) and homologous strains were typed by random amplified polymorphic DNA(RAPD) in the 39 MRSE strains.Results: The test of resistant genes in the 39 MRSE strains showed that occurrence of mecA,ermB,ermC,msrA,tetM,tetK,Aac(6′)/aph(2″),aph(3′)-III and mupA were 100.0%(39/39),12.8%(5/39),94.9%(37/39),43.6%(17/39),5.1%(2/39),2.6%(1/39),82.1%(32/39),25.6%(10/39) and 12.8%(5/39),and the resistant genes of ermA,tetO,tetL,ant(6′)-I,vanA and vanB were negtive.The 39 MRSE strains were divided into 11 types by RAPD and type A accounted for 48.7%(19/39).Conclusion: There were high percentage of mecA,ermC and Aac(6′)/aph(2″) resistant genes in MRSE and type A MRSE strains were the major prevalent strains in our region.
