经胸超声心动图在经胸微创房间隔缺损封堵术中的应用

摘　　要：目的:评价经胸超声心动图(TTE)指导经胸微创房间隔缺损(ASD)封堵术的可行性及作用。方法:32例ASD病人,年龄3～59岁,平均20±15.6岁,术前用GE VIVI7D彩色多普勒超声诊断仪经胸测量ASD的大小及周缘。ASD封堵术中监测封堵整个过程,封堵器的牢固性,观察有无残余分流。结果:术前用TTE测量的ASD最大径4～32mm,平均16.7±6.3mm,选用的封堵器是8～40mm,平均24.48±7.3mm,TTE测量的ASD大小与封堵器大小相关良好(r=0.94,P<0.001)。所有ASD病人封堵器固定牢固,无脱落。结论:TTE可准确测量ASD的大小,是指导ASD封堵的可行、有效、简便的方法。Objective： To evaluate the feasibility and effect of transthoracic echocardiography on guiding the occlusion of atrial septal defects by minimally invasive way.Methods：ASD size and its rim of thirty-two patients with of ASD,age ranged from 3 to 59（20±15.6）year,were measured by use of TTE with GE Vivid7 Color Doppler systems on various views before occlusion.occluding processes and steadiness of occluder werw monitored and whether residual shunt was left detected by color Doppler imaging during occlusion.Results：The size of ASD measured by TTE before occlusion was 4~32（16.7±6.3）mm,the size of selected occluder was 8~40（24.48±7.3）mm.Fine correlation was existed between the size of ASD measured by TTE and the size of selected occluder（r = 0.94 P0.001）.Occluder was firmly fixed without falling of all patients.Conclusion：TTE can used to accurately measure the size of ASD and is feasible effective and simple method for guiding ASD occlusion.
