浙江省岱山县居民死因分析

摘　　要：目的为掌握浙江省岱山县海岛地区居民健康状况和主要疾病死亡原因,评价居民健康水平,为政府和相关部门制定疾病预防控制策略提供参考依据。方法对岱山县1986-1988年和2009-2010年两个时期居民死亡资料进行比较分析,采用国际疾病分类法进行编码,以2000年全国标准人口构成比进行死亡率标化。结果岱山县两个时期居民粗死亡率分别为657.46/10万和767.65/10万,标化死亡率分别为699.37/10万和399.07/10万,两个时期死亡率均男性高于女性(u=6.17,P<0.01;u=7.48,P<0.01);前5位死因顺位依次分别为恶性肿瘤、呼吸系病、损伤中毒、脑血管病及消化系病和恶性肿瘤、呼吸系病、脑血管病、心脏病及损伤中毒,5类死因占死亡总数分别为77.74%和84.33%。居民平均期望寿命男性分别为68.58岁和75.80岁,女性分别为72.17岁和81.76岁,平均期望寿命分别上升了7.22岁和9.59岁,两个时期均为女性大于男性。结论岱山县居民以恶性肿瘤、呼吸系病、脑血管病、心脏病及损伤中毒为主要死因,其中肿瘤死亡率以肝癌居首,其次为胃癌。Objective To understand the health status of the residents in Daishan and the major death causes among them,evaluate the health level of the residents and provide evidence for disease prevention and control.Methods The statistical analysis and comparison were conducted on the death cause data in the residents in Daishan from 1986 to 1988 and from 2009 to 2010.The international classification of diseases was used for death cause coding and the national standard population data in 2000 was used for standardizing mortality.Results The crude mortality in the residents was 657.46/lakh and 767.65/lakh respectively during the two periods,and the adjusted mortality was 699.37/lakh and 399.07/lakh.The adjusted mortality during 2009-2010 was lower.The mortalities in males were higher than those in females during the two periods（u=6.17,P0.01,u=7.48,P0.01）.The first five death causes were malignant tumor,respiratory disease,injury/poisoning,cerebrovascular disease and digestive disease during 1986-1988,and malignant tumor,respiratory disease,cerebrovascular disease,heart diseases and injury/poisoning during 2009-2010,accounting for 77.74% and 84.33% of the total deaths respectively.The average life expectancy was 68.58 years and 75.80 years in males and 72.17 years and 81.76 years in females,which increased by 7.22 years and 9.59 years respectively.The average life expectancies in females were higher than those in males.Conclusion The major death causes in residents in Daishan were malignant tumor,respiratory disease,cerebrovascular disease,heart disease and injury/poisoning.Liver cancer was the leading death cause of cancer mortality,followed by gastric cancer.
