伴有慢性躯体疾病的老年抑郁症

摘　　要：目的了解躯体疾病对老年抑郁症的影响及伴有慢性躯体疾病的老年抑郁症患者的临床特征。方法对照分析38例有慢性躯体疾病的与无慢性躯体疾病的老年抑郁症病例。结果两组在临床症状和疗效上有显著差异,伴有慢性躯体疾病的老年抑郁症临床症状重,在精神焦虑症状(χ2=7.93,P<0.05)、易激惹(χ2=6.61,P<0.05)、自杀行为(χ2=9.77,P<0.05)、躯体不适(χ2=9.05,P<0.05)和疑病观念(χ2=4.29,P<0.05)症状上两组有显著差异;在近期疗效上疗效差,有显著差异(χ2=6.75,P<0.05)且治疗困难。结论慢性躯体疾病会影响老年抑郁症的临床症状与疗效。Objective To study the influence of physical disease of old age depression,and the clinical characteristic of elderly depression.Methods Compared 38 cases of chronic physical disease with no chronic physical disease of the elderly depression cases.Results The two groups in the clinical symptoms and effects were significant different,with physical disease of elderly depression had symptoms weight,on the anxious symptom of the mind（χ2=7.93,P0.05）,spirited（χ2=6.61,P0.05）,suicide behavior（χ2=9.77,P0.05）,physical discomfort（χ2=9.05,P0.05）and hypochondria concept（χ2=4.29,P0.05）symptoms,two groups had significant differences;On the near future curative effect poor efficacy,had significant differences（χ2=6.75,P0.05）and difficult treatment.Conclusions Chronic somatic diseases will affect the elderly depression symptoms and the clinical efficacy.
