摘  要:目的探讨西维来司钠(sivelestatsodium)雾化吸入对重度脑损伤大鼠肺的影响,为重度脑损伤患者实施肺保护提供理论基础。方法50只sD大鼠随机分为五组:正常组(A组)、重度脑损伤组(B组)、生理盐水对照组(C组)、地塞米松雾化吸入组(D组)、西维来司钠雾化吸入组(E组),每组10只。HE染色光镜观察肺组织病理改变;酶联免疫吸附试验(ELISA)法测定支气管肺泡灌洗液(BALF)的中性粒细胞弹性蛋白酶(NE)和白细胞介素-8(IL-8)含量;免疫透射比浊法测定BALF中白蛋白浓度。结果除正常组外,各组HE染色均可见不同程度的肺组织损伤,雾化吸人后E组改善最明显,其次为D组;B组BALF中NE、IL-8和白蛋白含量均高于其他组(P〈0.05),治疗干预后D、E组均有所降低(P〈0.05),E组更明显。结论西维来司钠雾化吸入对重度脑损伤大鼠肺组织有一定的保护作用,其机制可能与抑制NE产生和活性,减轻NE对肺组织的直接损伤和抑制肺内炎性循环反应导致的肺损伤有关。Objective To study the protective effect of sivelestat sodium by atomization inhalation on the lung of severe brain injury rats in order to provide the experimental evidences for the implementation of lung protection in severe brain injury (SBI) patients. Methods 50 SD rats were randomly divided into five groups : normal group ( A ), SBI group ( B ), saline control group ( C ), dexamethasone atomization inhalation group (D), sivelestat sodium atomization inhalation group (E), 10 each group. The pathological changes of lung tissue were observed by optical microscope. The levels of neutrophil elastase (NE), IL- 8 were measured in bronchoalveolar lavage fluid (BALF) by ELISA and the albumin concentration was detected by the immune turbidimetry. Results Except the normal group, the lung tissues of rats in all groups showed various degrees of lung injury. Group E was significantly improved after atomization inhalation, followed by group D. The NE, IL - 8 and albumin in the BALF were higher in group B than in other groups ( P 〈 0. 05 ), those in group D, E reduced obviously, especially in group E after therapy (P 〈 0.05 ). Conclusion Sivelestat sodium by atomization inhalation has a positive effect on the lung of severe brain injury rats. Its mechanism can be explained that sivelestat sodium may inhibit the production and activities of NE, alleviate the direct injury of lung tissue induced by NE and inhibit the lung injury caused by the inflammatory reaction of the lung.
