摘　　要：目的 了解深圳市福田人民医院临床分离对碳青霉烯类抗生素耐药革兰阴性杆菌产金属β-内酰胺酶的情况.方法 收集2010年6月～12月临床分离碳青霉烯类耐药菌株.用EDTA双纸片增效试验筛选分离的耐药细菌中的产金属β-内酰胺酶株,增效试验阳性菌株用聚合酶链反应（PCR） 扩增并通过对阳性扩增产物测序来确证金属酶基因型.结果 16株亚胺培南或美罗培南耐药细菌中铜绿假单胞菌10株,鲍曼不动杆菌4株,大肠埃希菌2株.双纸片增效试验阳性6株,PCR扩增阳性3株,1株VIM-2型和2株IMP-4型.结论 临床已出现产VIM-2型和IMP-4型金属β-内酰胺酶的碳青霉烯类抗生素耐药菌株,应加强对细菌耐药性监测及院内感染的管理控制,防止耐药菌株播散.Objective To investigate the genotypes of metallo-β-lactamase-producing bacteria isolated from Shenzhen Futian Hospital. Methods EDTA double pieces efficiency method was used to screen imipenem or meropenem drug-resistant bacteria and PCR and sequencing was used to identify metallo-β-lactamase genotypes of positive strains. Results A total of 16 imipenem or meropenem drug-resistant bacteria were collected,including 10 Pseudomonas aeruginosa,4 Acinetobacter baumannii and 2 Escherichia coll. 6 of which were positive by tested with EDTA double pieces efficiency test,3 strains respectively producing VIM-2,IMP-4 and IMP-4 were confirmed by PCR. Conclusion There were VIM-2 and IMP-4 metallo-β- lactamase-producing bacteria infection in the hospital. Resistance monitoring of clinical isolates should be strengthened to prevent spread of pathogens.
