异基因造血干细胞移植患者血清中AKP和-GT水平的变化及临床意义

摘　　要：目的 观察异基因造血干细胞移植（allogeneic hematopoietic stem cell,Allo-HSCT）病人血清中碱性磷酸酶（AKP）和γ-谷氨酰转肽酶（γ-GT）术前术后的变化及其与预后的关系.方法 检测45例中南大学湘雅医院异基因干细胞移植病人（即观察组）和65例该院体检正常者（即对照组）血清中AKP和γ-GT的水平,并与对照组比较,分析观察组术前、术后外周血清AKP和γ-GT水平的变化及与预后的相关性.结果 ①移植前,观察组的AKP平均浓度（88.93±48.09）u/L和γ-GT平均浓度（24.37±16.77）u/L与对照组的AKP平均浓度（83.25±14.36）u/L和γ-GT（24.08±9.78）u/L平均浓度均正常,且两组之间差异无统计学意义（P〉0.05）;②预后良好的观察组移植后的AKP平均浓度（80.11±17.62）u/L和γ-GT平均浓度（36.87±24.70）u/L均正常,与移植前比较差异无统计学意义（P〉0.05）;③预后差的观察组移植后的AKP平均浓度（183.87±220.24）u/L和γ-GT平均浓度（154.39±208.01）u/L明显升高,与移植前比较差异有统计学意义 （P〈0.05）.结论 患者血清中AKP和γ-GT水平可作为异基因造血干细胞移植术后预后判断的生化指标,对临床治疗及指导用药有一定辅助意义.Objective To observation the chang and its prognosis relationship of serum alkaline phosphatase （AKP） and - glutamyl peptide （γ-GT） between pro-operation and post operation in the petiants taken allogeneic hematopoietic stem cell transplantation. Methonts Totally 45 cases of Xiangya hospital patients after allogeneic hematopoietic stem cell transplanta- tion （observation group） and 65 cases of the hospital patients with normal physical examination （control group） were determined serum AKP and γ-GT value. Analysed the change and its prognosis correlation between pro-operation and post-operation. Results （1） Both the average,concentration of AKP （88.93±48.09）u/L,γ-GT （24.37±16.77）u/L in observation group and theaverage concenteation of AKP （83.25±14.36）u/L,γ-GT （24.08±9.78）u/L in control group were normal, and no significant difference in the two group （P〉0.05）. （2） The average concentration of AKP （80. 11± 17.62）u/L,γ-GT （36.87±24.70）u/l. after transplantation in the observation group that had good prognosis were normal,wire no statistical significance compared with per-transplantation （P〉0. 05）. （2） The average concentration of AKP （183.87±220. 24）u/L,γ- GT （154.39±208.01）u/L after transplantation in the observation group had poor prognosis were much higer, which had statistical significance compared with pre-transplantation （P〈0. 05）. Conclusion The serum AKP andγ-GT could act as useful biochemical markers which had prognosis significance hematopoietic stem cell transplantation, and play a secondary role in clinical treatment and drug guidance.
