经尿道绿激光气化术治疗高危前列腺增生疗效观察

摘　　要：目的:探讨经尿道绿激光气化术治疗高危前列腺增生患者的治疗效果及近期并发症。方法:采用经尿道绿激光气化术治疗143例高危前列腺增生患者,观察手术时间、术中出血量。手术前、后尿流率、国际前列腺症状评分(IPSS)、生活质量评分(QOL)、腹部B超测定剩余尿量(RUV)、直肠B超检查测定前列腺体积。术后冲洗时间、留置导尿管时间、近期并发症。结果:全部患者手术顺利进行,术中无并发症,术后停导尿管后有7例排尿困难,再留置导尿2周后拔除排尿顺畅,余患者排尿顺畅。术后下尿路感染9例,抗感染治疗后治愈。无呼吸衰竭、心力衰竭、心肌梗死、脑血管等并发症发生,所有患者均康复出院。术后IPSS、QOL、RUV、前列腺体积明显降低,最大尿流率明显提高。结论:经尿道绿激光前列腺汽化术治疗高危前列腺增生患者安全可行,有效。Objective： To investigate the effect of transurethral laser vaporization prostatectomy on high-risk benign prostatic hyperplasia.Methods：A total of 143 high-risk benign prostatic patients were selected,and all were treated by transurethral laser vaporization prostatectomy.The operation time,hemorrhage volume during surgery,the urinary flow rate,the International Prostate Symptom Score（IPSS）,the quality of life（QOL）,residual volume,volume of the prostate,the flush time,lasting time of catheterization after operation and the short term complication were observed.Results：The surgery was successful for all patients,without any complication.There were 7 cases with dysuria,who had recovered after two weeks by urethral catheterization.Nine cases had urinary tract infection and were healed after anti-infection treatment.No complications such as respiratory failure,heart failure and myocardial infarction occurred.IPSS,QOL,RUV and volume of the prostate were significantly decreased,and maximum urinary flow rate（MFR） increased significantly after operations.Conclusions：Transurethral laser vaporization prostatectomy is safe and effective in treatment of high risk benign prostatic hyperplasia.
