伽马钉治疗股骨粗隆间骨折疗效观察

摘　　要：目的分析伽马钉治疗股骨粗隆间不稳定骨折的疗效。方法选取我院2004年6月—2010年12月40例股骨粗隆间骨折患者,按Boyd和Griffin分型,Ⅰ型7例,Ⅱ型20例,Ⅲ型7例,Ⅳ型4例,Ⅴ型2例。采用闭合复位,透视下穿针,伽马钉内固定。结果 40例经4～12个月随访,38例患者均获得骨愈合,2例因其他原因死亡。骨折愈合时间9～15周,平均6周可持拐行走,生活基本自理。平均术后5.3个月能达较坚强愈合而负重。疗效按照功能评定标准评定:15例评定总分在39～44分为优,14例在28～38分为良,8例在18～27分为可,3例在18分以下为差,其优良率为72.5%。结论伽马钉治疗股骨粗隆间骨折,创伤小,手术时间短,恢复快,疗效满意,是治疗此类骨折的有效方法。Objective To explore the clinical results of treatment of Gamma-Nail for unstable intertrochanteric fracture of femur.Methods From 2004 to 2010,40 cases of unstable intertrochanteric fracture of femur were treated with Gamma-nails.According to Boyd and Griffin classification,7 cases were of stage Ⅰ,20 of Ⅱ,7 of Ⅲ,4 of Ⅳ,2 of Ⅴ.The treatment involved close reduction,insertion of guide pin and Gamma-nail fixation.Results The patients have been followed up for 4 to12 months（mean 6 months）.29 patients had excellent or good hip function,the excellent rate was 72.5%.Conclusion Gamma-nail fixation is an effective way to treat intertrochanter fracture of femur with less injury,shorter operation time and rapid recovery.
