耳内境下鼓膜置管联合左氧氟沙星滴耳液治疗分泌性中耳炎疗效分析

摘　　要：目的:探讨耳内镜下鼓膜置管联合盐酸左氧氟沙星滴耳液治疗分泌性中耳炎的临床疗效。方法:将符合标准的74例分泌性中耳炎患者随机分成观察组(43例)和对照组(31例),对照组采用耳内镜下鼓膜置管治疗,观察组在采用耳内镜下鼓膜置管治疗的基础上加用左氧氟沙星滴耳液治疗,分析两组的临床疗效。结果:观察组总有效率为95.35%,对照组总有效率为77.42%,两组患者的临床疗效经统计学处理,比较差异有统计学意义(P<0.05)。观察组治疗前细菌培养阳性率为41.86%,治疗后17例清除,1例未清除,细菌清除率为99.44%;对照组治疗前细菌培养阳性率为41.93%,治疗后9例清除,4例未清除,再感染1例,细菌清除率为69.23%,观察组与对照组细菌清除率差异有统计学意义(χ2=3.93,P<0.05)。观察组的分泌性中耳炎的复发率、感染率和耳漏发生率明显少于对照组,比较差异有统计学意义(P<0.05)。结论:耳内窥镜下鼓膜置管术联合左氧氟沙星治疗分泌性中耳炎效果良好,细菌清除率高,能够显著降低复发率和感染率,值得临床推广应用。Objective： To investigate the effect of endotoscope assisted tympanotomy tube insertion combined with levofloxacin on secretory otitis media.Methods： A total of 74 patients with secretory otitis media were randomly divided into observed group（43 cases） and control group（31 cases）.Patients in control group only received tympanotomy tube insertion under endotoscope,while patients in observation group received tympanotomy tube insertion combined with levofloxacin.The curative effect was observed and compared.Results：Total effective rate was 95.35% in the control group,and 77.42% in control group,with significantly difference between two groups（P0.05）.Positive rate of bacterial culture before treatment was 41.86% in observation group,and 41.93% in control group.After treatment,17 cases was removal which bacterial clearance rate was 94.29% in observation group,and 9 cases was cleared which bacterial clearance rate was 69.23% in control group.The difference was statistically significant in bacterial clearance rate（P 0.05）.The relapse rate,infection rate and the incidence of otorrhea in observation group were significantly lower than those of control group（P0.05）.Conclusions：Endotoscope assisted tympanotomy tube insertion combined with levofloxacin is effective on treating secretory otitis media,with high bacterial clearance rate.It can significantly reduce the relapse rate and infection rate,so is worthy of clinical application.
