产程活跃期使用间苯三酚加速产程进展的临床观察

摘　　要：目的观察产程活跃期使用间苯三酚促宫颈口扩张,加速产程进展的作用。方法选择2008年1月—2010年12月住院分娩的足月妊娠的单胎头位初产妇,宫口开大≥3 cm伴宫颈质韧或水肿者185例随机分成A、B、C 3组:A组(62例)在产程活跃期静脉注射间苯三酚80 mg,B组(63例)在产程活跃期静脉缓慢注射654-2 10 mg,C组(60例)在产程活跃期静脉缓慢注射地西泮10 mg(>5 min)促宫颈口扩张,观察3组产程进展、分娩方式、胎儿宫内窘迫、新生儿Apgar评分及产后出血情况。结果间苯三酚组用药至宫口开全时间为(3.4±1.2)h,第1产程平均时间为(8.2±1.7)h,明显短于654-2组与地西泮组,差异有统计学意义(P<0.05)。结论产程活跃期使用间苯三酚可消除宫颈水肿,促宫口扩张,加速产程进展,效果优于654-2及地西泮,而且对母儿无不良影响。[Objective]To observe the effect of phloroglucinol on improving cervical dilatation and promoting the labor progress during active phase of labor. [Methods]185 normal term primipara with single pregnancy,head position,cervical dilatation greater than or equal to 3 cm,cervical toughness or cervical edema were collected from January 2008 to December 2010.They were randomly divided into three groups.In order to improving cervical dilatation,group A（62 cases） was treated with phloroglucinol 80 mg by intravenous injection during active phase of labor,group B（63 cases） was treated with 654-2 10 mg by intravenous injection slowly,and group C（60 cases） was treated with diazepam 10 mg by intravenous injection slowly（5 min）.The labor progress,delivery mode,fetal distress and newborns Apgar score of three groups were observed. [Results]The duration from drug administration to full dilation of the cervix in group A was（8.2±1.7） h,and the average duration of the first stage of labor was（8.2±1.7） h,which were significantly shorter then than those in group B and group C（P0.05）. [Conclusion]Phloroglucinol can alleviate the cervical edema,improve cervical dilatation and accelerate labor progress during active phase of labor,its effect is better than 654-2 and diazepam,and without adverse effect on mothers and fetuses.
