摘 要:目的探讨河南省有偿供血人群HIV/HCV混合感染者体内HIV-1细胞嗜性的分布情况。方法选取河南省经有偿供血途径感染HIV-1的161例感染者,采用酶联免疫法检测HCV抗体,并通过HCV核酸检测明确HIV/HCV混合感染。nested-PCR扩增env C2V3区,测序并分析HIV-1流行亚型,使用在线预测工具WebPSSM预测辅助受体的使用情况,分析V3环顶端四肽。结果河南省经有偿供血途径感染HIV-1人群的抗HCV阳性率为87.58%(141/161),HCV RNA阳性率为56.52%(91/161),HIV单独感染组和HIV/HCV混合感染组HIV-1均以B(B')亚型多见(96%和98.5%),V3环顶端四肽主要为GPGR(33.4%和33.8%)、GPGQ(22.0%和19.1%)和GPGK(14.0%和19.1%)。2组感染者的HIV-1均以使用CCR5辅助受体为主,HIV-1细胞嗜性在2组中的分布差异无统计学意义(P=0.971)。结论 HCV混合感染不影响HIV-1的细胞嗜性。HCV RNA检测有助于明确HIV/HCV混合感染者现症感染情况。Objective To investigate HIV-1 cell tropism of the population coinfected with HIV and HCV through paid blood donation in Henan province.Methods Totally 161 HIV-1 sero-positive blood samples were collected from individuals who were coinfected with HIV and HCV through paid blood donation in Henan province.HCV antibody was determined using enzyme-linked immunosorbent assay,and HIV/HCV coinfection was confirmed by the detection of HCV RNA.HIV-1 env C2V3 gene was amplified using nested-PCR and the subtype characterization was analyzed after sequencing.Co-receptor usage was predicted using biosoftware WebPSSM,and distribution of tetramer compositions on the tip of V3 loop was analyzed.Results Among the total 161 individuals infected with HIV-1 through paid blood donation,HCV sero-prevalence was 87.58%(141/161),and the positive rate of HCV RNA was 56.52%(91/161).The most predominant HIV-1 subtypes in both HIV monoinfection group and HIV/HCV coinfection group were B(B')(96% and 98.5%,respectively),and the main tip motifs of V3 loop were GPGR(33.4% and 33.8%),GPGQ(22.0% and 19.1%) and GPGK(14.0% and 19.1%).Co-recepter CCR5 was mainly used in both HIV monoinfection group and HIV/HCV coinfection group,and the HIV-1 cell tropism between the two groups was not significantly different(P=0.971).Conclusions The cell tropism of HIV-1 has no correlation with HCV infection.The decetion of HCV RNA helps to clarify the current infection situation in HIV/HCV coinfected population.
