摘 要:目的探讨超声检查及引导穿刺在AIDS相关恶性肿瘤和机会性感染患者中的临床应用价值。方法对108例AIDS合并恶性肿瘤和机会性感染患者的超声表现及其中94例经活组织细胞学穿刺、引流的情况进行回顾性研究。结果在108例中,恶性肿瘤27例,其中淋巴瘤15例,卡波西肉瘤5例,子宫颈癌5例,子宫颈上皮内瘤变2例;机会性感染81例,其中结核性感染68例,其他病菌性感染13例。5例肝脓肿均临床治愈。结论 AIDS并发恶性肿瘤以非霍奇金淋巴瘤多见,其次是卡波西肉瘤;女性患者多见子宫颈癌及宫颈上皮内瘤变;机会性感染以结核性感染为多,或多种感染并存;超声检查可发现多处病灶,引导穿刺可提供可靠的病理诊断依据,具有重要临床应用价值。Objective To investigate the clinical value of ultrasonic examination and ultrasonography-guided puncture in patients with AIDS-related malignant tumor and opportunistic infections.Methods Ultrasonic manifestations of 108 patients with AIDS-related malignant tumor and opportunistic infections and the data of 94 patients who had been given biopsy and cytological puncture and drainage were retrospectively studied.Results Among 108 patients,27 had malignant tumor,among whom 15 had lymphoma,5 Ka-posi's sarcoma,5 uterine cervix cancer,and 2 uterine cervical intraepithelial neoplasia;81 had opportunistic infections,among whom 68 had tuberculous infection,and 13 other bacterial infection.Five patients with hepatic abscess were cured clinically.Conclusions Hodgkin lymphoma is the most common in AIDS patients with malignant tumor,followed by Kaposi's sarcoma;uterine cervical cancer and cervical intraepithelial neoplasia are commonly seen in female patients;tuberculous infection and co-existence of multiple types of infections are prevalent in AIDS patients with opportunistic infections;ultrasonic examination can detect multiple lesions,and ultra-sonography-guided puncture can provide reliable pathological diagnosis,which has important clinical application value.
