摘 要:传统腹膜透析(PD)液的高浓度葡萄糖、高葡萄糖降解产物、高糖基化终产物、低pH值等生物不相容性是影响PD疗效、导致PD技术失败的主要原因。因此研制生物相容性更好的透析液,已成为改进PD质量的重要内容。使用中性pH值、低葡萄糖降解产物(neutral pH、low-GDP,NpHLGDP)的透析液可延长腹膜寿命、提高疗效。本文主要针对NpHLGDP透析液的生物相容性、评价生物相容性的生物标志物及可能的临床预后作一简述。The development of peritonitis, the decline of residual kidney function, and the loss of peritoneal membrane function are central events for patients with peritoneal dialysis and affect technique survival. These are closely correlated with the poor biocompatibility of conventional peritoneal dialysis (PD) solutions containing high concentration of glucose,glucose degradation products (GDPs) coupled with the hyperosmolarity, reduced pH, and use of lactate as the buffer. Therefore, the use of neutral pH, low-GDP (Npl-ILGDP), bicarbonate-buffered PD solutions may represent a promising strategy to attenuate some of these adverse effects. In this article, we will highlight evidence regarding the biocompatibility of NpHLGDP PD solutions and compare the advantage and disadvantage of NpHLGDP solutions with traditional PD solutions, review the utility of current biomarkers in the evaluation of biocompatibility, and discuss the clinical outcome data with these solutions.
