社区高血压患者药物治疗依从性及自我效能分析

摘　　要：目的探讨广州市社区高血压患者药物治疗依从性及自我效能现状,并分析二者的相关性,为制定有效的社区高血压管理策略提供科学依据。方法随机抽取广州市3个城市社区≥18岁的高血压患者576例,采用Morisky问卷及自我效能量表进行问卷调查。结果社区高血压患者药物治疗依从性好者占38.7%,依从性差者占61.3%,其中55.2%的患者有忘记服药的经历。不同文化程度及不同经济收入的患者,其服药依从性间差异均有统计学意义(P<0.05)。依从性差的患者较依从性好的患者自我效能水平低,差异有统计学意义(P<0.05)。服药依从性得分与自我效能总分及各维度得分呈负相关(r总分=-0.581,r促进因子=-0.575,r障碍因子=-0.504,P均<0.01)。结论广州市区社区高血压患者药物治疗依从性较低,自我效能水平与依从性呈正相关关系,建议加强社区高血压患者的健康教育和用药的规范化管理,提高社区慢性病患者的管理水平。Objective To explore the present condition of medication compliance and self-efficacy of hypertension patients in community in Guangzhou,and to analyse their relationship between the two so as to provide scientific bases for strategies of hypertension management in communities.Methods 576 hypertension patients with or over 18 years old were selected randomly from 3 urban communities in Guangzhou.Medication compliance and self-efficacy were measured by the ＂Morisky questionnaire＂and the ＂self-efficacy scale＂.Results The rate of poor medication compliance in patients with hypertension was 61.3%,55.2% of which had the experience of forgetting to take medicine.There was significant difference in drug compliance between patients who had different educational background and economic income（P〈0.05）.The level of self-efficacy was lower in patients with poor compliance than patients with good compliance,and statistical significance was found（P〈0.05）.A negative correlation was found between medication compliance scores and self-efficacy scores（r=-0.581,-0.575,-0.504;P0.01）.Conclusion The medication compliance of hypertension patients in communities was relative low in Guangzhou,The level of self-efficacy had a positive correlation with the level of medication compliance.Therefore,it is suggested to strength health education and standardized management of hypertension patients in communities,and to improve the management level for patients with chronic diseases in communities.
