摘 要:目的评价非机械通气患者经口/鼻采用自制二氧化碳采样器连续无创监测PETCO2的临床应用效果。方法非机械通气患者50例,用自制二氧化碳采样器经口/鼻吸氧,连续监测和比较不同氧流量下PETCO2图形和读数变化,分析恒定氧流量下PETCO2与PaCO2的相关性。结果不同氧流量下均可检测到正常形态PETCO2矩形波形;不同氧流量下不同时间点所测得的PETCO2数值差异均无统计学意义。PETCO2与PaCO2呈中度相关(r=0.50~0.55,P<0.05)。结论自制呼气末二氧化碳采样器经口/鼻连续无创监测PETCO2,在不同氧流量下均具有较好的稳定性,同时具有经济、安全、无创、实用的特点,可以作为危重病人抢救时的快速监测指标之一。Objective To evaluate the clinical application of a home-made carbon dioxide sampler in continuous monitoring trans-oral/nasal PrrCO2 in patients with non-mechanical ventilation. Methods Fifty non-ventilated patients inhaled O2 using a home-made carbon dioxide sampler through which trans-oral/nasal PETCO2 waveform and readings were continuously monitored and were compared under different flow rates of O2. Meanwhile, the correlation between PETCO2 and PaCO2 at constant flow rate of O2 was analyzed. Results The normal square wave of PETCO2 could be detected under different flow rates of O2 and the differences between PETCO2 values under different flow rates or at different time points were not significant. A medium correlation was found between PETCO2 and PaCO2(r=0.50-0.55,P〈0. 05). Conclusion This home-made carbon dioxide sampler can continuously monitor trans-oral/nasal PETCO2 and has a stable performance under different flow rates of O2. It is economic, safe, noninvasive and practical, and can be used as a rapid monitoring index for therapeutic efficacy in rescuing critical patients.
