两组Snodgrass尿道下裂修补的对比研究

摘　　要：目的评价Snodgrass法尿道下裂修补法及在不同条件下施术的结果。方法回顾2003年9月至2005年3月在津巴布韦中央医院（国外组27例）和2005年6月至2010年7月在国内（国内组24例）采用Snodgrass法施行尿道下裂修补的资料。国外组未行阴茎包皮脱退、转移皮瓣覆盖新尿道，缝线粗，单通道导尿管引流，术后3～5d出院。国内组术前准备充分，游离阴茎包皮皮瓣并转移覆盖新尿道，7／0～9／0可吸收线缝合，Foley’s尿管可靠引流，抗生素预防感染1～2周。结果国外组伤口裂开漏尿6例，阴茎轻度弯曲3例，无需再矫形治疗。需要再手术的发生率22．3％。国内组伤口裂开漏尿1例，术后出血1例，加压包扎后出血停止。暂时漏尿2例，随访时痊愈。再手术的发生率4．2％。结论Snodgrass尿道下裂修补显著优于其他修补法。合适的手术条件是获得预期效果的根本。Objective To evaluate the result of Snodgrass tubularised incised plate hypospadias repair and the importance of interrelated conditions around the operation. Methods Two groups of patients underwent Snodgrass tubularised incised plate hypospadias repair. The patients in overseas group were from Zinbabwe （27cases） and the other patients in native group were from China（24 cases）. The conditions of native group including preoprative preparation,use of antibiotic, demand of suture and urinary catheter, urethral covering using vascularized dorsal sub- cutaneous tissue were superior to those of overseas group. Results The fistula formation occured in 6 pts. in overseas group. The rate of reoperation was 22.3%. A light bend of penis occurred in 3 patients but no need of operation. In the native group, the fistula formation appearred in 1 patient and the rate of reoperation was 4.3%. No bend of penis was seen. Conclusion Good cosmetic and function might be expected using Snodgrass tubularised incised plate hypospadias repair under suitable conditions around the opration
