血浆纤维蛋白原水平与冠状动脉病变程度的关系

摘　　要：目的 探讨冠心病患者血浆纤维蛋白原（Fbg）水平与冠状动脉病变程度的相关性.方法 选择进行冠状动脉造影的375名患者,每位患者均于术前空腹抽血进行生化及Fbg等指标检测.根据造影结果分为正常对照组、一支病变组、两支病变组和三支病变组.采用Genisi＇s评分系统对冠心病患者病变程度进行评分.结果 4组血浆Fbg 水平分别为：对照组（2.19±0.34）g / L、一支病变组（2.64±0.73）g / L、两支病变组（2.85±0.66）g / L、三支病变组（3.19±0.80）g / L,两两比较差异均有统计学意义（r=0.494,P＜0.05）;Gensini＇s评分分组与Fbg水平的关系显示：随着冠状动脉病变程度的增加,Fbg水平增加且差异有统计学意义（r=0.576,P＜0.05）,多因素回归分析显示血浆Fbg水平与病变程度明显相关（P＜0.01）.结论 冠心病患者血浆Fbg水平同冠状动脉病变支数及病变程度呈正相关,多支病变患者及冠状动脉病变程度的增加血浆Fbg升高.Objective To investigate the correlation of plasma levels of fibrinogen （Fbg） with the severity of coronary artery lesions. Methods Total 375 patients underwent coronary arteriongraphy from Aug 2008 to Dec 2010. The patients were divided into 4 groups according to number of diseased coronary arteries, and the severity was evaluated with Genisi scoring system. The plasma levels of fasting glucose, fibrinogen, creatinine, CK-MB were measured. Results The plasma levels of Fbg in subjects with normal coronary arteries, one branch lesion, two branch lesion and three branch lesion were 2.19 ± 0.34, 2.64 ± 0.73, 2.85 ±0.66 and 3.19 ± 0.80 （g/L） respectively （r=0.494, P〈0.05）. Plasma Fbg levels were significantly correlated with Geneisi scores （r=0.576, P〈0.01）. Conclusion The plasma fibrinogen levels are correlated with the number and severity of coronary arterial lesions.
