摘 要:目的 研究缺氧诱导因子-1α(HIF-1α)和PTEN蛋白在子宫内膜异位症(EMs)中的表达及其调节,从血管形成和基因调控方面进一步探究它们在EMs发生、发展中的作用和意义.方法 取EMs异位内膜58例,EMs在位内膜25例,正常子宫内膜20例,应用免疫组化二步法对HIF-1α和PTEN蛋白的表达进行研究,结合病例的治疗、复发及受孕情况进行分析.结果 HIF-1α在EMs患者的异位内膜中表达高于在位内膜和正常子宫内膜中的表达率差异均有统计学意义(均P<0.05),PTEN在EMs患者的异位、在位内膜低于正常子宫内膜中的表达(均P<0.05);HIF-1α在EMs在位和异位内膜中的增生期和分泌期中的表达比较差异有统计学意义(P<0.05),而在正常子宫内膜中差异无统计学意义(P >0.05);而PTEN在EMs患者在位、异位和正常子宫内膜中的增生期和分泌期中的表达差异无统计学意义(P >0.05).在HIF-1α和PTEN 蛋白表达与患者的年龄、痛经情况、囊肿大小及术后受孕与否均无相关性,但与临床分期和术后复发情况有相关性;HIF-1α和PTEN蛋白表达呈负相关,差异有统计学意义(P<0.05).结论 HIF-1α直接参与EMs的血管生成,HIF-1α可能是血管生成的核心调控因子.PTEN蛋白能够诱导EMs细胞凋亡,抑制其生长,它在EMs中表达存在缺失或减弱.它们相互作用对EMs细胞的异位种植和存活创造了条件.Objective To investigate the expression of HIF-1α and PTEN in endometriosis (EMs) and its clinical significance. Methods The expression of HIF-1α and PTEN were detected by immunohistochemical technique in 58 specimens of ectopic endometrium and 25 specimens of eutopic endometrium from 38 EMs patients, and 20 specimens of normal endometri- urn. The correlation of HIF-1α and PTEN expression with treatment, recurrence and pregnancy of patients was analyzed. Results The positive rate of HIF-1α in ectopic endometrium, eutopic endometrium of EMs patients was higher and PTEN was lower than those in normal endometrium (both P〈0.05 ). HIF-1α was significantly higher in secretary phase than that in proliferative phase both in ectopic and eutopic endometrium of EMs patients (P〈0.05),but not in normal endometrium. There was no difference in PTEN expression between secretary phase and proliferative phase of endometrium. The expression of HIF-1α and PTEN was not correlated with age, dysmenorrhea, cyst diameter, pregnancy of EMs patients, but correlated with clinical staging and recurrence after operation (P〈0.05). There was negative correlation between HIF-1α and PTEN expression in endometrium (P〈0.05). Conclusion The results suggest that the interaction of HtF-1α and PTEN may be involved in the ectopic implantation and development of endometriosis cells in EMs patients
