摘　　要：目的研究中国EV71感染手足口病(HFMD)中CD8+细胞毒性T细胞的活化和第二信号的表达,探讨EV71感染的病程发展机制。方法选取48例EV71HFMD患儿分为轻症组、重症组(其中包括中枢神经病变组、中枢神经病变伴自主神经失调组、神经源性肺水肿组),应用流式细胞术检测CD8+T细胞上CD28和CD38的表达水平,分析CD8+T细胞的质量水平在EV71感染病程发展中的作用。结果 CD8+CD28+细胞、活化分子CD38的表达轻症组与健康同龄对照组、重症各组比较增高明显,差异有统计学意义(P<0.05)。结论 CD8+细胞毒性T细胞在EV71感染中的反应质量可能是病程发展的原因之一。Objective To study the CD8＋ cytotoxic T cell activation and the expression of second signal in patients with EV71 infection,to explore the mechanism of EV71 infection progression.Methods The 48 children patients with EV71 HFMD were divided into mild group,severe group that included central nervous system lesions group,central nervous system lesions associated with autonomic nervous dysfunction group,neurogenic pulmonary edema group PE.The levels of CD28 and CD38 expression on CD8＋T cells were detected with flow cytometry,the quality of CD8＋ T cells were analyzed in the disease progression of EV71 infection.Results The CD8＋CD28＋cells,the expression of CD38 activation molecule in mild group increased significantly compared with control group,severe groups,the difference was statistically significant（P0.05）.Conclusion Quality of CD8＋ cytotoxic T cells in the EV71 infection may be one of the reasons in the disease progression.
