腹腔镜用于胰腺外科的体会

摘　　要：目的:探讨腹腔镜技术用于胰腺外科的价值。方法:回顾分析2007年9月至2010年8月24例患者施行腹腔镜胰腺手术的临床资料,其中15例行腹腔灌洗引流术,5例行腹腔镜胰十二指肠切除术,2例行胰腺部分切除术,1例行胰腺体尾部切除术,1例行胃空肠侧侧吻合术。结果:手术时间0.5～10 h,平均5.7 h。术中出血量5～300 ml,平均135 ml。术后第1天腹腔引流量15～80 ml,平均35 ml,胃管引流量50～520 ml,平均123 ml。排气时间1～5 d,平均2 d。住院7～48 d,平均22 d。结论:腹腔镜技术在胰腺外科领域发挥了微创优势,值得推广应用。Objective：To discussion the clinical application value of laparoscopic in pancreatic surgery.Methods：From Aug.2007 to Sep.2010,24 cases with pancreatic diseases were treated by laparoscopic.Including 15 cases of lavage and draindag,5 cases of LPD,2 cases of partial-pancreatectomy,1 distal pancreatectomy and 1 stomach-jejunostomy were included.Results：All of the operations were completed successfully with an average operating time of 5.7 h（rang 0.5-10 h） and a mean intraoperative blood loss of 135 ml（rang 50-520 ml） The median postoperative abodominal drainage of the first-day was 35 ml（rang 15-80 ml）.The median postoperative hospital stay was 22 d（rang 7-48 d）.Conclusions：The laparoscopic surgery for pancreatic diseases is minimally invasive,which should be reserved.
