采用处方评价表评价门诊处方用药合理性

摘　　要：目的根据《处方管理办法》中处方评价表评价中山大学附属第一医院门诊处方的用药合理性。方法 根据处方评价表进行数据统计,对不同时段的处方进行对比分析。结果分析本院门诊处方共3 109张,并分别按季度比较。平均每张处方用药品种数最多为3种,最少为2.34种;平均每张处方金额最高为283.1元,最低为141.23元;抗菌药物使用平均疗程最长为6.34 d,最短为3.99 d。抗菌药物使用百分率最低为5.08%,注射剂使用百分率最低为2.3%,中成药使用百分率最低为28.81%。结论《处方管理办法》及《医院处方点评管理规范(试行)》实施以来,药学部根据处方评价表对处方的用药合理性进行分析,并及时与医院主管部门沟通,处方质量较前得到明显提高。处方点评对促进合理用药,提高处方质量,保障医疗安全起到积极的指导作用。Objective To evaluate the rationality of the outpatient prescriptions in the First Affiliated Hospital of Sun Yat-Sen Universityby the evaluation form fromPrescription Management. Methods The data of the prescriptions were collected by the evaluation form, and prescriptions of different periods were compared and analyzed. Results A total of 3 109 prescriptions were analyzed according to the quarter of the year. The average number of varieties of drugs per prescription was up to 3 species, while at least 2.34 species. The highest average cost per prescription was 283.1 yuan, and the lowest of average cost was 131.23 yuan. The average course of treatment for antimicrobial drug application was up to 6.34 d, and minimum of 3.99 d. The lowest percentage of antimicrobial drug application was 5.08%, and the lowest percentage of injection application was 2. 3% , the lowest percentage of traditional Chinese medicine application was 28.81%. Conclusion Since implementation of Prescription Management and Hospital Prescription Practice Comments （Trial）, the evaluation form is adopted to analysis the rationality of prescriptions. Department of Pharmacy communicates with the hospital authority timely, and the quality of prescriptions is significantly promoted. Evaluation form can promote the rational prescription drugs, and improve the quality of prescriptions, and it can play an active role in guiding the health safety.
