摘 要:探讨前列腺素E1(prostaglandin E1,PGE1)对肾缺血再灌注损伤(ischemiareperfusion injury,IRI)大鼠血中性粒细胞(polymorphonuclear neutrophils,PMNs)细胞间黏附分子-1(intercellular cell adhesion molecule-1,ICAM-1)水平和肾组织血栓调节蛋白(thrombomodulin,TM)表达的影响。健康Wistar大鼠80只随机分为空白对照组、假手术组、模型组、PGE1治疗组,每组按不同术后观察时间(1、3、6、24 h)均分。观察造模后其血清肌酐(creatinine,Cr)水平变化,采用流式细胞术和实时定量聚合酶链反应方法(Rt-PCR)同步监测ICAM-1表达水平和肾组织TM mRNA表达的动态变化。(1)模型组IRI损伤1 h,Cr水平虽然没有明显变化,但血清ICAM-1水平及肾小管坏死评分值已较假手术组明显增加(P均<0.05),并与肾组织TM mRNA表达升高的变化峰值时点相一致。(2)IRI损伤6 h,模型组和治疗组CrI、CAM-1水平均明显增高(P<0.05),但治疗组CrI、CAM-1水平及肾小管坏死积分值均明显低于模型组(P<0.05)。(3)肾组织中TM mRNA表达与肾小管损伤积分值呈现良好的相关性(r=0.69,P<0.01);且与ICAM-1水平的增加呈现良好的相关性(r=0.62,P<0.05)。(1)ICAM-1T、M mRNA表达增多,反映了IRI内皮细胞的损伤程度;(2)PGE1通过减少ICAM-1及TM mRNA水平的表达,阻抑凝血及炎症反应过程,保护内皮细胞,减轻肾组织损伤。To approach the effect of prostaglandin E1(PGE1) on the expression of polymorphonuclear neutrophils(PMNs) intercellular cell adhesion molecule-1(ICAM-1) in blood and the thrombomodulin(TM) in nephridial tissue of ischemia reperfusion injury(IRI) rats.80 healthy Wistar rats were randomized into blank control,sham-operated control,IRI control and PGE1 treated groups.Meanwhile,each group was stratified further according to the postoperative observation periods(1,3,6 and 24 h).The serum creatinine(Cr) level after modeling was observed.Expressions of ICAM-1 both in mRNA level and cellular level were measured by flow cytometry and real time polymerase chain reaction(Rt-PCR).Also dynamic state changes of TM mRNA level in nephridial tissue was monitoringed by Rt-PCR meanwhile.Results showed that(1)Compared with sham-operated control group,Cr levels had no obvious change at 1h after IRI in IRI control group,while serum ICAM-1 level and renal tubular necrosis score increased markedly(P0.05) and was identical with the upregulated expression peak of TM mRNA in nephridial tissue.(2)6 h after IRI,both Cr and ICAM-1 levels were increased significantly(P0.05) in IRI control and PGE1 treated groups.While levels of Cr,ICAM-1 and the renal tubular necrosis score in PGE1 treated group were significantly lower than that of IRI control group(P0.05).(3)There were good correlations between the expression of TM mRNA in nephridial tissue and renal tubular necrosis score(r=0.69,P0.01),also it was well correlated to the increase of ICAM-1level(r=0.62,P0.05).The increase of ICAM-1 and TM mRNA levels reflect the degree of endotheliocyte injury in IRI.PGE1 can depress the process of blood clotting and inflammatory reaction to protect endotheliocyte and lessen nephridial tissue injury by down-regulate levels of ICAM-1 and TM mRNA.
