摘　　要：目的:研究慢性乙型肝炎和肝衰竭患者HBeAg的表达及其与血清中HBV-DNA的关系。方法:收集天津市传染病医院2007年4月—2010年9月收治的118例慢性乙型肝炎患者和109例肝衰竭患者确诊时的相关资料,分析2组患者HBV-DNA含量的变化、HBeAg阳性和阴性的比例,以及HBeAg阳性和阴性患者的HBV-DNA含量的变化。结果:肝衰竭组HBeAg阴性患者所占比例较慢性乙型肝炎组高,且HBeAg阴性组HBV-DNA水平较阳性组低;与慢性乙型肝炎组相比,肝衰竭组HBV-DNA水平较低,HBeAg阳性和阴性的肝衰竭患者HBV-DNA水平均低于相应的慢性乙型肝炎组患者;慢性与不同分期的慢加急性(亚急性)肝衰竭患者HBeAg阴性所占比例差异无统计学意义(P>0.05),慢性肝衰竭和慢加急性(亚急性)肝衰竭早期组患者的HBV-DNA水平低于慢加急性(亚急性)肝衰竭晚期组,而慢加急性(亚急性)肝衰竭中、晚期患者HBV-DNA水平差异无统计学意义(P>0.05)。结论:HBeAg的不同表达在肝衰竭和慢性乙型肝炎病情演变过程中可能起着重要作用,但不影响病情的轻重。Objective：To explore the expression of HBeAg and the relationship between HBeAg and serum HBV-DNA in patients of chronic hepatitis B（CHB） and liver failure.Methods：The data in this study were collected from 118 patients with CHB and 109 patients with liver failure in Tianjin Infectious Disease Hospital from April 2007 to September 2010.The changes of HBV-DNA content,the proportion of HBeAg-positive and-negative patients and content changes of HBV-DNA of HBeAg-positive and-negative patients were analyzed retrospectively.Results：In liver failure group,the proportion of HBeAg-negative patients was higher than that of CHB patients,and the HBV-DNA level of HBeAg-negative patients was lower than that of HBeAg-positive patients.The level of HBV-DNA was lower in liver failure group than that of CHB group.The levels of HBV-DNA were lower in HBeAg positive and negative patients with liver failure than those of patients with CHB.There were no significant differences in proportions of HBeAg-negative patients between patients with chronic liver failure and early acute-on-chronic liver failure（P 0.05）.The HBV-DNA level was lower in patients with chronic liver failure and early acute-on-chronic liver failure than that of patients with late acute-on-chronic liver failure.But there was no significant difference in the level of HBV-DNA between medium and late acute-on-chronic liver failure patients（P 0.05）.Conclusion：The expression of HBeAg may play an important role in the evolution of chronic hepatitis B and liver failure,but it is not related to the severity of illness.
