初诊2型糖尿病患者动脉血管受损的超声观察

摘　　要：目的应用彩色多普勒超声技术检测初诊2型糖尿病患者肱动脉血管内皮功能及颈动脉、足背动脉粥样硬化的状况并探讨其相关性。方法对50例初诊2型糖尿病患者与对照组非2型糖尿病患者30例,超声检查颈动脉、足背动脉及测定反应性充血时和舌下含服硝酸甘油后肱动脉内径的变化,并对其进行相关性分析。结果糖尿病组和对照组,在加压和口服药的情况下与静息状态下相比血管内径均增大,但糖尿病组内径变化率均明显低于对照组(P<0.01,P<0.05),差异有统计学意义。糖尿病组颈动脉、足背动脉IMT较对照组明显增厚,差异有统计学意义(P<0.01)。糖尿病组颈动脉异常率、足背动脉异常检出率明显高于对照组;糖尿病组足背动脉异常检出率明显高于颈动脉异常。结论采用彩色多普勒超声技术联合检测肱动脉皮功能及颈动脉、足背动脉粥样硬化是评价糖尿病血管异常的有效方法,对早期诊断、预防及治疗具有重要的临床应用价值。Objective To probe the correlation between atherosclerosis of carotid and dorsal and the endothelial functionin patients with newly diagnosed diabetic mellitus by using Color Doppler Ultrasound.Methods Atherosclerosis of carotid and dorsal and the changes of brachial artery internal diameter under the reactive hyperemia situation or sublingual nitroglycerin in 50 patients with newly diabetes and 30 patients without diabetes were observed and their correlation was analyzed.Results There was a statistically significant difference between experiment group and control group（P0.01,P0.05） in the expanding degree of brachial artery internal diameter under reactive hyperemia situation or sublingual nitroglycerin.IMT in patients with diabetes was pachynsed significantly（P0.01） than that of control group.In 50 patients with newly diabetes there was a notable difference between atherosclerosis of carotid and dorsal.The patients with atherosclerosis of dorsal were more than those with atherosclerosis of carotid.Conclusion The combined use of branchial endothelial function and atherosclerosis of carotid and dorsal by color Doppler ultrasonography may be a effect method to evaluate arterial disease in patients with diabetes.
