摘 要:目的对肿瘤坏死因子α(TNF-α)、环氧合酶2(COX-2)和p38丝裂原活化蛋白激酶(p38MAPK)进行相关性分析,研究TNF-α/p38MAPK/NF-κB/COX-2信号通路在鼻息肉发病中的作用机制,以深入阐明鼻息肉的发病机制,为临床治疗鼻息肉及寻找特异性治疗药物提供理论依据。方法选取20例未采用糖皮质激素治疗的复发性鼻息肉组织标本、20例初发鼻息肉组织标本、20例正常鼻黏膜组织。采用苏木素-伊红(HE)染色和免疫组织化学染色、常规反转录聚合酶链反应(RT-PCR)、Western印迹实验对鼻息肉和正常鼻黏膜组织中的TNF-α、COX-2和p38MAPK进行检测。结果 TNF-α、COX-2和p38MAPK在正常下鼻甲黏膜组织中无或极少量表达,在鼻息肉组织中均呈高表达(P<0.01)。复发型鼻息肉组织中,TNF-α、COX-2和p38MAPK的阳性表达面积和密度均高于鼻息肉组(P<0.05)。鼻息肉组中,TNF-α、COX-2和p38MAPK的表达成正相关(P<0.05)。结论 TNF-α、COX-2和p38MAPK在鼻息肉组织中均呈高表达,而且在复发性鼻息肉组织中表达更显著。p38MAPK信号通路参与鼻息肉的发生、发展,可能起偶联作用。Objective To explore the expression of tumor necrosis factor α (TNF-α) ,cyclooxygenase 2 (COX-2)and p38 mitogen-activated protein kinases(p38MAPK) in nasal polyps, and to find the role of TNF-α/p38MAPK/ NF- KB/COX-2 in the process of nasogenesis. Methods Specimens were acquired from patients with palindromia nosogene- sis (n = 20), primary nasogenesis (n = 20) and 20 normal nasal tissues. The expressions of TNF-α,COX-2 and p38MAPK in nosogenesis and normal tissues were detected by hematoxylin-eosin(HE) staining, immunohistochemistry ,RT-PCR and western blot. Results TNF-α,COX-2 and p38MAPK expressed higher in nosogenesis group than in control group,and especially in the palindromia nosogenesis. The expression of TNF-α and COX-2 had a positive correlation with p38MAPK. Expression of COX-2 in nosogeuesis tissue cut down markedly after using budesonide. Following increase of budesonide, expression of COX-2 and NF-KB took on a dose-dependent depression. Conclusions TNF-α,COX-2 and p38MAPK expressed notablely higher in nasogenesis group than in control group, and especially in the palindromia naso- genesis group. Expression of TNF-α and COX-2 had a positive correlation with p38MAPK. It indicated that p38MAPK signal pathway played a role in the development and progression of nasal polyps.
