摘  要:目的探索超声诊断杵状指的方法。方法 10例杵状指患者,10例健康体检者为对照组,观察杵状指的形态学变化,测量指甲下软组织前后径(SDUN),远端指间关节处的前后径(IPD)及甲床处手指的前后径(DPD),并计算DPD/IPD。结果超声可清楚观察指甲、指甲下软组织、指骨、指节关节以及指甲延伸到软组织内部分。杵状指组SDUN较对照组增大(P<0.01)。杵状指组DPDs大于其IPDs,而对照组的DPDs小于其IPDs。杵状指组DPD/IPD大于对照组(P<0.01)。结论超声诊断杵状指较其他方法简便,SDUN增大也许可作为诊断杵状指的早期指征。Objective To explore the method of high frequency ultrasound in diagnosis of digital clubbing. Methods Ultrasonic examination was performed on 10 patients with digital clubbing and 10 healthy control subjects, the morphology of the clubbing fingers was observed. The soft tissue depth under the nail ( SDUN), the interphalangeal depth (IPD) and the distal phalangeal depth (DPD) were measured, then the ratio of DPD and IPD (DPD/IPD) was calculated. Results The nails, SDUN, the digital bones and the digital joint were clearly observed by uhrasound including the part of nails inserting into the soft tissue. The SDUN in clubbing group was increased than that in control group (P〈0.01) , DPDs were higher than the IPDs in clubbing group , while DPDs were lower than the IPDs in control group. The ration of DPD/IPD in clubbing group was higher than that in control group (P〈0.01). Conclusion Uhrasonography is a new, easy and convenient method for diagnosis of dizital clubbinz. The increased SDUN might be taken as an early sign of digital clubbing.
