摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者外周血单个核细胞(PBMC)分泌的TNF-α、白细胞介素(IL)-8和IL-10水平变化以及与肺功能的相关性。方法分离COPD急性加重期(n=30)患者治疗前后和健康对照组(n=30)PBMC,采用酶联免疫吸附法(ELISA)测定其分泌TNF-α、IL-8和IL-10水平及其与第1秒用力呼吸量占预计值的百分比(FEV1/pre%)相关性。结果 COPD急性加重期组PBMC分泌TNF-α和IL-8水平高于治疗后组和对照组(P<0.05),治疗后组高于对照组(P<0.05);而IL-10水平在COPD急性加重期组低于治疗后组和对照组(P<0.05),治疗后组低于对照组(P<0.05)。COPD急性加重期组、治疗后组患者TNF-α、IL-8均与FEV1/pre%呈明显负相关(P<0.05);IL-10与FEV1/pre%呈明显正相关(P<0.05)。结论 COPD气道炎症与TNF-α、IL-8和IL-10水平不平衡有关,检测COPD患者PBMC分泌的TNF-α、IL-8和IL-10水平可作为判断COPD炎性反应程度的指标。Objective To investigate the correlation between the changes of levels of tumor necrosis factorct ( TNF-α ), interleukin-8 ( IL-8 ), interleukin-α ( IL-10 ) from peripheral blood mononuclear cells (PBMC) in patients with chronic obstructive pulmonary disease (COPD) and pulmonary function. Methods The peripheral blood mononuclear cells (PBMCs) were isolated from 30 healthy subjects and 30 patients with COPD at acute exacerbation period, and the levels of TNF-α, IL-8 and IL-10 were detected by ELISA, and related inflammatorycytokines and FEV1/pre% were measured as well. Results The levels of TNF-ot, and IL-8 in patients with COPD at acute exacerbation period were significantly higher than those in posttreatment group and control group ( P 〈 0.05 ), and the levels of IL-10 in posttreatment group were significantly higher than those in control group (P 〈 0.05 ) ,however,the levels of IL-10 in patients with COPD at acute exacerbation period were significantly lower than those in posttreatment group and control group ( P 〈 0.05 ), which in posttreatment group were significantly lower than those in control group ( P 〈 0.05 ). The levels of TNF-α, IL-8, IL-10 in patients with COPD at acute exacerbation period and in posttreatment group were negatively correlated with FEV1/pre% ( P 〈 0.05 ) , however, the levels of IL-10 were positively COiTelated with FEV1/pre% ( P 〈 0.05 ). Conclusion The airway inflammation is related with the imbalance of levels of TNF-α, IL-8 and IL-10, and the detection of levels of TNF-α,IL-8 and IL-10 can be used as the indexes in judging the inflammatory reaction degree in patients with COPD.
