不同营养比例的支持治疗对慢性阻塞性肺疾病患者营养状况的影响

摘　　要：目的探讨不同营养比例的支持治疗对慢性阻塞性肺疾病(chronic obstructive pulmonary diseases,COPD)患者营养状况的影响。方法选取2010年1—12月在湖州市中心医院治疗的60例COPD患者作为研究对象。按随机数字表法将其分成3组,即对照组、蛋白组、脂肪组,每组各20例。按不同营养比例制定相应食谱,比较各组试验前、试验后3个月、试验后6个月患者的身体质量指数(body mass index,BMI)、上臂围(mid-upper arm circumference,MAC)、上臂皮褶厚度(triceps skinfold,TSF)、上臂肌围(arm muscle circumference,AMC)、血清转铁蛋白(transferrin,TRF)、血清前白蛋白(prealbumin,PAB)等变化。结果 3组患者在试验前后各项指标的差异均有统计学意义(P<0.05)。试验后3、6个月蛋白组各项指标均优于对照组、脂肪组,差异有统计学意义(均P<0.05)。结论恰当的营养支持治疗能改善患者的营养状况,适当提高蛋白质比例对COPD患者各项营养指标的提高更为有效。有效的饮食管理是确保营养支持治疗效果的必要手段。Objective To study the influence of nutritional supportive treatment by different proportion to the nutritional status in patients with chronic obstructive pulmonary diseases （COPD）. Methods From January 2010 to December 2010,60 patients with COPD were divided randomly into 3 groups： control group（N=20） ,protein group（N=20）,fat group（N=20）,with corresponding recipe according to different proportion of nutrition. The differences in body mass index（BMI）, mid-upper arm circumference（MAC）, triceps skinfold（TSF） ,arm muscle circumference（AMC） ,serum transferring（TRF） and prealbumin（PAB） were compared before experiment and at 3 months,6 months after experiment. Results There was a statistical significance on the indicators before and after experiment（P〈0. 05）. The indicators at 3 months, 6 months after experiment were superior to in protein group to those of control and fat groups（P〈0.05）. Conclusion Appropriate nutrition therapy can improve the nutritional status of patients. An adequate increase in the proportion of protein in patients with COPD also improves the nutrition indicators effectively. Effective dietary regulation plays an vital role in ensuring the therapeutic efficacy of nutritional supportive treatment.
