癌症伴抑郁患者心理防御特点研究及护理干预

摘　　要：目的 探讨癌症伴抑郁患者心理防御特点及护理对策.方法 采用抑郁自评量表（SDS）及防御方式问卷（DSQ）对70例癌症患者进行测试,根据SDS抑郁指数将患者分成研究组（癌症伴抑郁组）和对照组（癌症不伴抑郁组）,比较两组患者的防御方式.并从防御机制人手,对抑郁患者加强心理分析,给予综合护理干预.结果 两组患者SDS粗分及标准分间差别有统计学意义（P＜0.001）；DSQ因子中不成熟型防御机制得分间差别有统计学意义（P＜0.001）；不成熟防御机制之投射、被动攻击、潜意显现、抱怨、退缩、躯体化得分,成熟型防御机制之升华,中间型防御机制之否认、交往倾向、消耗倾向、期望得分间差别均有统计学意义（P＜0.05）.防御方式由不成熟型向成熟型转化有利于病情的改善.结论 癌症伴抑郁患者多使用不成熟型防御方式,与癌症不伴抑郁患者防御机制不同.通过护理干预可减轻患者的抑郁程度,有利于患者的综合治疗,提高患者的生活质量.Objective To explore the characteristics of psychological defense of cancer patients with depression and nursing intervention countermeasure.Methods Totaled of 70 cancer patients were tested by the questionnaires of SDS and DSQ and divided into two groups：study group and control group.Defense styles of two groups were compared..Results There were statistical differences between two groups in crude scores and standard scores of SDS （P 〈0.001 ）.Mature defense style was helpful to improving the disease.Conclusions Cancer patients with depression use more immature defense style,and depression in patients with cancer defense mechanism is not different.Nursing intervention can reduce depression in patients,be beneficial to cancer patients with combined therapy,improve patients＇quality of life.
